Required Credentials and Documentation
Submission Requirements (Must be submitted before assignment)
- Current nursing license with primary source verification (within 30 days prior to start)
- Nursys license verification (within 30 days prior to start)
- Driver’s License or State ID (must not be expired, must match address on file)
- Resume/Profile (Ascension Resume and Profile)
- At least two professional references
Onboarding Requirements (Must be completed before or at start)
- Ascension Cerner Test-Out (Cerner test results and training checklist with signed attestation)
- Ascension Badge Photo (forward-facing headshot with white or blue background, no alterations)
- Health Statement or completed physical exam (updated annually)
- Fit Test (preferred before start, can be completed onsite; specific mask types required)
- Documentation of vaccinations and immunizations:
- MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
- Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
- Hepatitis B: Vaccination series completion or signed declination
- Tdap: Valid within 10 years (no declinations accepted)
- Influenza Vaccine (optional, but if declined during flu season, mask required)
- 2 Step PPD or IGRA testing for TB (with annual TB questionnaire)
- Positive PPD requires negative chest X-ray within 1 year and annual TB questionnaire
- Ascension 12 Panel Drug Screen (within 30 days prior to start, updated annually)
- Criminal Background Check (including Social Security Trace, Employment Verification, OIG Search, National Sex Offender)
- Employment Verification for past 7 years
- Education Verification relevant to clinical work (within 90 days prior to start)
- Ascension Skills Checklist (clinical skills, updated annually)
- Ascension Specialty Exam (minimum 80% passing score, updated every 3 years)
- Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80% score, updated yearly)
- Ascension EKG Exam (minimum 80% passing score, updated as required)
- Ascension Color Vision Test (updated every 3 years)
- BLS Certification (AHA)
- ACLS Certification (AHA)
- NIH Certification (updated yearly or every 2 years depending on group)
- CPI/MOAB High Risk Online Portion (completed prior to start)
- Ascension Onboarding Requirements Education Attestation
- Ascension Nursing Contract Worker Onboarding (done onsite)
- Ascension Nova StatStrip Pre-Hire Modules
Optional Credentials
- Influenza Vaccine Exemption/Declination (mask required if declined during flu season)

This is a 13-week ICU RN contract position at Ascension Via Christi St. Francis in Wichita, Kansas.
