Job Opportunity: RN CVICU - Nashville, Tennessee
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Start Date: January 5 (Non-negotiable)
Shift Hours: 36 or 48 hours per week
Required Credentials and Certifications
Licenses & Verification (Required for Submission)
- Current nursing license with primary source verification within 30 days prior to start.
- Nursys verification within 30 days prior to start.
- Driver's License or State ID (must not be expired and must match address on file).
- Professional resume/profile.
- At least two professional references.
- Badge photo (forward-facing headshot with white or blue background, no alterations).
Certifications (Required for Onboarding)
- BLS (American Heart Association)
- ACLS (American Heart Association)
- NIH Certification (Group A updated yearly; Groups B-F every 2 years)
Health & Immunizations (Required for Onboarding)
- MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted).
- Varicella: Documented proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted).
- Tdap: Valid within 10 years (no declinations accepted).
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Fit Test: Required for all travelers; preferred prior to start, can be completed onsite.
- 2 Step PPD or IGRA (Quantiferon/Tspot): Recent testing required with annual TB questionnaire.
- Positive PPD requires negative chest X-ray within 1 year and TB questionnaire.
- Health Statement or physical exam prior to first assignment, updated annually.
- 12 Panel Drug Screen within 30 days prior to start, updated annually.
Onboarding Requirements (Must be completed prior to or at start)
- Ascension Core Competencies: HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, two attempts allowed.
- Ascension EKG Exam: Passing score of at least 80%, two attempts allowed.
- Ascension Onboarding Education Attestation: Completion of 6 hours pre-hire education.
- Ascension Nursing Contract Worker Onboarding: Completed onsite.
- Ascension Nova StatStrip: Pre-hire modules for non-allied positions.
- Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation.
- Ascension Skills Checklist: Clinical skills checklist completed within one year, updated annually.
- Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually.
- EPLS (Excluded Parties List System) check within 90 days of start.
- Education Verification: Relevant clinical education verified within 90 days of start.
- Employment Verification: Past 7 years verified; updated annually.
- Confirmation of Assignment: Provided upon initial assignment and updates for extensions.
Optional
- Ascension Unit Specific Checklist (uploaded by unit leader within a week).
- Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire (optional if not positive).
- Hiring Manager Evaluation (post-assignment).
- Legal Name Change Documentation.
This position requires strict adherence to credentialing, health, and onboarding requirements to ensure compliance and readiness for assignment start.
