Location: Nashville, Tennessee
Employment Type: Long-Term Assignment (LTA)

Shift Hours: 36 or 48 hours per week
Contract Length: 13 weeks


Required Credentials & Onboarding Requirements

Licenses & Certifications

  • Nursys License Verification: Must be verified within 30 days prior to start. Complete FACIS Questionnaire if any disciplinary actions.
  • Current License/Certificate Verification: Primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
  • BLS Certification: Must be American Heart Association (AHA) certified.
  • NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
  • Driver's License/State ID: Must be current, show front and back, and address must match records.

Health & Immunizations

  • MMR: Proof of two vaccines or positive IGG titer required. No declinations accepted. If negative, two doses four weeks apart required.
  • Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider. No declinations accepted. If negative, two doses four weeks apart required.
  • Tdap: Valid within 10 years. No declinations accepted.
  • Hepatitis B: Documentation of vaccination series completion or signed declination.
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31). Exemptions accepted but mask required at all times.
  • Fit Test: Required for all travelers; preferred before start but can be done onsite. Annual update required.
  • 2 Step PPD or IGRA (Quantiferon/Tspot): First PPD within 1 year, second within 30 days of start, or negative IGRA within 90 days. Annual TB questionnaire required. Positive TB requires chest X-ray within 1 year.
  • Positive PPD: Negative chest X-ray and annual TB questionnaire required (optional for submission).
  • Health Statement: Statement of good health or completed physical exam prior to first assignment; updated annually.

Testing & Exams

  • Ascension Medical Surgical Exam: Must be completed prior to start and updated every 3 years. Minimum 80% score; only 2 attempts allowed.
  • Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics. Completed within a year of start and updated yearly. Minimum 80% score; only 2 attempts allowed.
  • Ascension EKG Exam: New edition as of Dec 1, 2023. Passing score of at least 80% required; only 2 attempts allowed.
  • Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required prior to start.
  • Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start; updated annually.
  • Ascension 12 Panel Drug Screen: Within 30 days prior to start and updated annually. Includes testing for 12 specified drugs. Retesting required if break in service > 90 days.

Onboarding & Documentation

  • Ascension Resume/Profile: Required for submission.
  • Ascension 2 Professional References: At least two qualifying references required prior to first assignment.
  • Ascension Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check. Completed within 30 days of first assignment and updated annually or after break in service > 90 days.
  • Ascension EPLS (Excluded Parties List System): Run within 90 days of start; rerun if break in service > 90 days.
  • Ascension Education Verification: Verification of education relevant to clinical work within 90 days of start; rerun if break in service > 90 days. Official transcripts required if verification fails.
  • Ascension Employment Verification: Third-party verification for past 7 years; updated annually or after break in service > 90 days.
  • Ascension Confirmation of Assignment: Provided upon initial assignment and updated with each extension. Must reflect accurate start and end dates.
  • Ascension Onboarding Requirements Education Attestation: Completion of 6-hour pre-hire module with attestation upload required.
  • Ascension Nursing Contract Worker Onboarding: Completed onsite.
  • Ascension Nova StatStrip: Pre-hire modules required for non-allied positions prior to clearance.
  • Ascension Badge Photo: Forward-facing headshot with white or blue background; no alterations or filters.

Optional / Additional

  • Ascension Influenza Vaccine Exemption/Declination: Accepted with mask requirement during flu season.
  • Ascension Unit Specific Checklist: Optional for onboarding; must be uploaded by unit leader within a week.
  • Hiring Manager Evaluation: Post-assignment evaluation (optional).
  • Ascension Name Change Document: Legal documentation of name change (optional).
