Job Opportunity: RN Labor and Delivery (Travel)
Location: Hoffman Estates, Illinois

Facility Information
- Labor and Delivery Unit
- Located in Hoffman Estates, IL
Required Certifications & Licenses
- BLS (Basic Life Support) – Must be AHA certified
- ACLS (Advanced Cardiovascular Life Support) – Must be AHA certified
- NRP (Neonatal Resuscitation Program) – Must be AHA/American Academy of Pediatrics certified
- Current Nursing License – Nursys verification within 30 days prior to start; primary source verification required
- License/Certificate Verification – Current and verified within 30 days prior to start
Required Exams & Testing
- Ascension Pharmacology Exam – Must be completed prior to start; updated every 3 years; minimum 80% score; max 2 attempts
- Ascension Specialty Exam – Completed prior to first assignment and updated every 3 years; minimum 80% or national benchmark passing score; max 2 attempts
- Ascension Core Competencies – Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly; minimum 80% score; max 2 attempts
- Ascension Cerner Test-Out – Cerner test results and training checklist with signed attestation required
- Ascension Skills Checklist – Clinical skills checklist to be completed within one year of start and updated annually
Health & Immunization Requirements
- MMR (Measles, Mumps, Rubella) – Documented proof of two vaccines or positive IGG titer; no declinations accepted
- Hepatitis B – Documentation of vaccination series completion or signed declination
- Influenza Vaccine – Required for flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
- Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire – Must provide positive IGRA or PPD results, chest X-ray within a year, and completed TB questionnaire; updated annually
- 2 Step PPD OR IGRA (Quantiferon/Tspot) – Required within 30 days prior to start
- Varicella – Proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted
- Tdap – Valid within 10 years; no declinations accepted
- Fit Test – Required for all travelers; preferred prior to start, can be completed onsite; updated annually
- Health Statement – Statement of good health or completed physical exam prior to first assignment; updated annually
- Ascension 12 Panel Drug Screen – Completed within 30 days prior to start; includes testing for 12 specified drugs; must include MRO review if applicable
Onboarding & Professional Requirements
- Ascension Onboarding Requirements Education Attestation – Completion of pre-hire education attestation required
- Ascension Nursing Contract Worker Onboarding – Onsite completion required
- Ascension Biometric Consent – Required
- Ascension Nova StatStrip – Pre-hire modules for non-allied positions required prior to clearance
- Driver's License/State ID – Must be current, not expired, and address must match licensing requirements
- Ascension Profile (Resume) – Required for submission
- Ascension 2 Professional References – At least two professional references required prior to first assignment
- Ascension Criminal Background Check – Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender; completed within 30 days prior to first assignment and updated annually
- Ascension EPLS (Excluded Parties List System) – Run within 90 days of start; rerun if break in service > 90 days
- Ascension Education Verification – Verification of education relevant to clinical work; completed within 90 days of start; rerun if break in service > 90 days
- Ascension Confirmation of Assignment – Provided upon initial assignment and updated with extensions
- Ascension Employment Verification – Third-party verification for past 7 years; updated annually; rerun if break in service > 90 days
Optional / Additional Information
- Ascension Unit Specific Checklist – Optional for onboarding but must be uploaded by unit leader within a week
- Hiring Manager Evaluation – Post-assignment evaluation (optional)
- Ascension Name Change Document – Legal documentation of name change (optional)
