Experience Requirements

Required Credentials & Certifications

• Ascension Core Competencies (HIPAA, Joint Commission, OSHA) – Completed within a year of start, updated yearly, minimum 80% score
• Ascension EKG Exam – Minimum 80% score, updated as per latest edition
• Ascension Onboarding Requirements Education Attestation
• Ascension Nursing Contract Worker Onboarding – Completed onsite
• MMR Vaccination or Positive IGG Titer – No declinations accepted
• Fit Test – Preferred prior to start, required annually
• Hepatitis B Vaccination or Declination Documentation
• Influenza Vaccine – Required during flu season (Oct 1 - Mar 31), exemptions accepted with mask requirement
• Positive PPD or Negative Chest X-Ray and Annual TB Questionnaire – Must provide documentation
• Health Statement or Physical Exam – Completed prior to first assignment, updated annually
• 2 Step PPD or IGRA Test – Within 1 year and 30 days of start
• Varicella Immunity or Vaccination – No declinations accepted
• Tdap Vaccine – Valid for 10 years, no declinations accepted
• 12 Panel Drug Screen – Within 30 days prior to start, updated annually
• Criminal Background Check – Including Social Security Trace, OIG Search, National Sex Offender, completed within 30 days of first assignment, updated annually
• EPLS (Excluded Parties List System) Check – Within 90 days of start, rerun if break in service > 90 days
• Skills Checklist – Clinical skills checklist completed within one year of start, updated annually
• Education Verification – Within 90 days of start, rerun if break in service > 90 days
• Employment Verification – Past 7 years, updated annually
• Driver’s License/State ID – Must be current and valid, address must match home state for compact license holders

Additional Information

• Forward-facing headshot with white or blue background required (no filters or alterations)
• Multiple Ascension-specific exams and checklists must be completed and passed with minimum scores as noted
• Some documents and attestations must be uploaded prior to or at onboarding
• Flu vaccine exemptions require mask wearing during flu season
