Experience Requirements

- Ascension Nursing Contract Worker Onboarding – Completed onsite
- MMR Vaccination or Positive IGG Titer – No declinations accepted
- Fit Test – Preferred prior to start, required annually
- Hepatitis B Vaccination or Declination Documentation
- Influenza Vaccine – Required during flu season (Oct 1 - Mar 31), exemptions accepted with mask requirement
- Positive PPD or Negative Chest X-Ray and Annual TB Questionnaire – Must provide documentation
- Health Statement or Physical Exam – Completed prior to first assignment, updated annually
- 2 Step PPD or IGRA Test – Within 1 year and 30 days of start
- Varicella Immunity or Vaccination – No declinations accepted
- Tdap Vaccine – Valid for 10 years, no declinations accepted
- 12 Panel Drug Screen – Within 30 days prior to start, updated annually
- Criminal Background Check – Including Social Security Trace, OIG Search, National Sex Offender, completed within 30 days of first assignment, updated annually
- EPLS (Excluded Parties List System) Check – Within 90 days of start, rerun if break in service > 90 days
- Skills Checklist – Clinical skills checklist completed within one year of start, updated annually
- Education Verification – Within 90 days of start, rerun if break in service > 90 days
- Employment Verification – Past 7 years, updated annually
- Driver’s License/State ID – Must be current and valid, address must match home state for compact license holders
Additional Information
- Forward-facing headshot with white or blue background required (no filters or alterations)
- Multiple Ascension-specific exams and checklists must be completed and passed with minimum scores as noted
- Some documents and attestations must be uploaded prior to or at onboarding
- Flu vaccine exemptions require mask wearing during flu season
