Required Qualifications

• License: Current RN license in Indiana with primary source verification within 30 days prior to assignment start.
• Certifications:

  • BLS (American Heart Association)
• Exams & Testing:

  • Pharmacology Exam (minimum 80%, updated every 3 years)
  • Medical Surgical Exam (minimum 80%, updated every 3 years)
  • EKG Exam (minimum 80%, updated every 3 years)
  • Core Competencies (HIPAA, Joint Commission, NPSG, OSHA; minimum 80%, updated every 3 years)
  • Skills Checklist (clinical specialty, completed within 1 year and updated annually)
• Health Requirements:

  • MMR: Proof of two vaccines or positive titer; no declinations accepted.
  • Varicella: Proof of two vaccines, immunity, or history; no declinations accepted.
  • Hepatitis B: Vaccination series completion or signed declination.
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement.
  • Tdap: Valid within last 10 years; no declinations accepted.
  • TB Testing: Positive IGRA or PPD results, negative chest X-ray within 1 year, and annual TB questionnaire.
  • Fit Test: Required annually; preferred prior to start.
  • Health Statement or physical exam prior to first assignment, updated annually.
  • 12 Panel Drug Screen within 30 days prior to start, updated annually.
• Professional Requirements:

  • Nursys verification within 30 days prior to start.
  • License/Certificate verification within 30 days prior to start.
  • Resume/Profile submission.
  • Two professional references prior to first assignment.
  • Criminal background check including Social Security Trace, OIG Search, National Sex Offender, and others within 30 days prior to start; updated annually.
  • EPLS check within 90 days prior to start; rerun if break in service > 90 days.
  • Education verification within 90 days prior to start.
  • Employment verification for past 7 years.
  • Driver’s License/State ID (must be current and valid).
  • Badge photo (white or blue background, no alterations).
• Onboarding Requirements:

  • Completion of Ascension Nursing Contract Worker Onboarding and Onboarding Education Attestation.
  • Confirmation of assignment documentation.
