Occupational Therapist Position - Tulsa, Oklahoma
Job Details
- Specialty: Occupational Therapist (OT)
- Location: Tulsa, Oklahoma
- Employment Type: Long-Term Assignment (LTA)
- Contract Length: 13 weeks
- Shift: 8:30 AM – 5:00 PM, Monday through Friday, occasional weekends
- Setting: 100% Acute Rehab in an acute hospital setting
- Floating: No floating to other rehab settings
- Experience Requirement: Must have acute care experience
Required Qualifications
- Valid Occupational Therapist license in Oklahoma
- Minimum of 2 years of Occupational Therapist experience
- Occupational Therapist experience within the last year
Required Credentials and Documentation
Professional:
- Resume/Profile submission required before assignment
- At least two professional references prior to first assignment
- Signed assignment agreement (offer letter and confirmations) required at onboarding
- Driver’s License or State ID (valid, not expired, matching address requirements)
- Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others completed within 30 days of start
- Employment verification for the past seven years
- Education verification relevant to clinical work completed within 90 days of start
- Skills checklist for specialty completed within one year of start and updated annually
- Confirmation of assignment documentation on agency letterhead at start and upon extensions
Personal Health History:
- MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
- Varicella: Proof of two vaccines, immunity, or verified history; no declinations accepted
- Tdap: Valid within 10 years; no declinations accepted
- Hepatitis B: Vaccination series completion or signed declination
- Influenza vaccine: Optional, but if declined during flu season (Oct 1 – Mar 31), mask wearing required
- Fit test: Required for all travelers, preferred before start, can be completed onsite
- TB testing: 2-step PPD or IGRA within specified timeframes; if positive, chest X-ray and annual TB questionnaire required
- Health statement or physical exam prior to first assignment, updated annually
- 12-panel drug screen within 30 days prior to start and annually thereafter
Additional Information
- Flu season mask policy applies if influenza vaccine is declined
- No floating to other rehab settings; strictly acute rehab
- Occasional weekend work may be required
- All documentation must be current and compliant with onboarding timelines
- Post-assignment surveys and evaluations may be requested but are optional
