Physical Therapist (PT) - Travel Position

Location: Cobleskill, New York
Contract Length: 12 weeks

Job Setting

• Primary outpatient care
• Inpatient acute and swing-bed rehab coverage (less frequent)

Patient Population & Case Types

• Outpatient:

  • General orthopedics and practice PT
  • Common diagnoses: shoulder, back, hip, knee
  • Falls/balance referrals
  • Neurological diagnoses
• Inpatient:

  • Critical-access hospital with a small swing-bed rehab unit (~15 patients)
  • Diagnoses include orthopedic injuries, post-surgical recovery, and general illness rehabilitation post-acute stay

Case Volume

• 15 sessions per day (30 minutes each)
• Evaluations: 1 hour each (limited to 4-5 per day)
• Follow-ups: 30 minutes each
• No double or over-booking in usual practice

Required Credentials & Certifications

• BLS (Basic Life Support)
• License/Certificate Verification (Physical Therapist license)
• Various facility-specific competencies and skills checklists
• Immunizations and health screenings including:

  • MMR
  • Fit Test
  • Hepatitis B
  • Influenza Vaccine
  • TB Test and Annual TB Questionnaire
  • Varicella
  • Drug Screen
• Driver's License/State ID
• Professional references (2 required)
• Criminal background check
• Resume and employment verification
• Education verification
