Required Credentials and Certifications

• BLS (Basic Life Support) – AHA or ARC only
• PALS (Pediatric Advanced Life Support) – AHA only
• ACLS (Advanced Cardiovascular Life Support) – AHA only
• TNCC (Trauma Nursing Core Course) – As required per specialty/unit & facility
• Nursys License Verification – Nursing license within 30 days of start
• License/Certificate Verification – Primary source verification within 30 days of start, must be active in Louisiana
• FMOLHS Skills Checklist – Unit-specific, within one year prior to start, updated annually
• FMOLHS Competency Exam – Passing score of 80% or higher within one year prior to start
• FMOLHS Core Competencies – Includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; updated annually

Personal Health Requirements

• COVID-19 Vaccine – Proof or exemption accepted (optional)
• MMR (Measles, Mumps, Rubella) – Proof of 2 immunizations, positive titer, or declination with signed statement
• Hepatitis B – Proof of 3 immunizations, positive titer, or declination with signatures and agency approval
• Influenza Vaccine – Seasonal requirement; medical/religious exemptions accepted with proper documentation (optional)
• TB Test – Negative QuantiFERON blood test within 1 year or 2-step skin test; if positive, MD note or proof of treatment required
• Health Statement – Signed and dated within 1 year stating ability to work without restrictions
• Varicella – Proof of 2 immunizations, positive titer, or declination with signed statement
• Tdap (Tetanus, Diphtheria, Pertussis) – Proof of vaccination within past 10 years or declination with signatures and agency approval
• Annual TB Questionnaire – Required within 6 months of start; if positive, documentation of evaluation and treatment completion required
• Fit Test – N95 mask fit test (3M 1860, 1860s, or 1870+) completed within 1 year prior to start, updated annually
• FMOLHS Eye Exam & Color Vision – Ishihara Color Test and Snellen Vision test within 1 year prior to start

• FMOLHS Resume – Required for submission
• FMOLHS Criminal Background Check Results – Must include SSN address trace, county and national criminal checks, education and employment verification (last 5 years), driving record, and various federal watch lists; completed within 30 days prior to start
• FMOLHS Drug Screen – Negative 10-panel drug screen within 30 days prior to start
• FMOLHS Badge Photo – Forward-facing, well-lit, neutral background, JPEG format
• FMOLHS Contract Annual Education – Required annually
• FMOLHS HRIS Form – Required for submission
