Job Opportunity: RN Critical Care
Location: Lafayette, Louisiana
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Required Credentials and Compliance for Onboarding
Certifications
- BLS: AHA or ARC only (required for onboarding)
- ACLS: AHA only (required for onboarding)
Licenses
- Nursys: Nursing license verification within 30 days of start (required for submission)
- License/Certificate Verification: Primary source verification of professional license/certification within 30 days of start, updated upon expiration. Must be active in Louisiana for this assignment (required for submission).
Personal Health History Requirements
- COVID-19 Vaccine: Proof of vaccine or exemption accepted (optional)
- MMR: Proof of 2 immunizations, positive titer, or declination with signed statement (required for onboarding)
- Fit Test: N95 mask fit test (3M 1860, 1860s, or 1870+) within 1 year prior to start, updated annually (required for onboarding)
- Hepatitis B: Proof of 3 immunizations, positive titer, or declination with required signatures (required for onboarding)
- Influenza Vaccine: Required during flu season (10/1 - 4/30). Medical or religious exemptions accepted with proper documentation and signatures (required for onboarding)
- TB Test: Negative QuantiFERON blood test within 1 year or 2-step skin test; if positive, must provide MD note or proof of treatment (required for onboarding)
- Health Statement: Signed and dated within 1 year stating ability to work without restrictions (required for onboarding)
- Varicella: Proof of 2 immunizations, positive titer, or declination with signed statement (required for onboarding)
- Tdap: Proof of vaccination within past 10 years or declination with required signatures (required for onboarding)
- Annual TB Questionnaire: Completed within 6 months of start; if positive, documentation of evaluation and treatment completion required (required for onboarding)
- Eye Exam & Color Vision: Ishihara Color Test and Snellen Vision test within 1 year prior to start (required for onboarding)
Background and Drug Screening
- Criminal Background Check: Must be completed within 30 days prior to start, including SSN address trace, county criminal searches for all counties lived/worked in last 7 years, national criminal check, education and employment verification (last 5 years), driving record, and multiple federal databases (required for onboarding)
- Drug Screen: Negative 10-panel drug screen within 30 days prior to start (required for onboarding)
Skills and Competency Testing
- Skills Checklist: Unit-specific checklist within 1 year prior to start, updated annually (required for onboarding)
- Competency Exam: Passing score of 80% or higher on required exams within 1 year prior to start (required for onboarding)
- Core Competencies: Must be current within 1 year prior to start with passing score of 80% or higher; includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age-Specific Care, Cultural Diversity, Advance Directives; renewed annually (required for onboarding)
