Key Requirements and Credentials

Certifications & Licenses

• CST Certification (NBSTA or NCCT only) required for Massachusetts locations (preferred for NH).
• Certified Registered Central Service Technician (CRCST or NBSTSA CST) required.
• Professional license and/or certification must be verified within 30 days of start date and updated upon expiration.
• MA Conditional Licenses are not accepted.

Health & Immunization

• MMR Vaccine: Documentation of two measles, two mumps, and one rubella vaccine OR two MMR vaccines, or proof of immunity via blood test.
• TB Test: Documentation of TB skin test or negative QuantiFERON TB Test or T-spot within 3 months of start date; must be updated if there is a break >6 months.
• Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start date; updated if >6 month break.
• Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions allowed.
• Hepatitis B Vaccine: Strongly preferred (3 doses or proof of immunity or declination).
• Tdap Vaccine: Preferred within last 10 years.
• Respirator Medical Clearance (RMC) Attestation: Required if fit testing is needed for the role.

Background & Professional Checks

• Criminal Background Check: Within 45 days prior to start date; updated if >6 month break. Includes county and international searches if applicable.
• Massachusetts CORI Check: Completed within 30 days prior to start date; updated if >6 month break.
• National Sex Offender Registry Check: Within 30 days of start and updated annually.
• Exclusion Checks: Includes OIG, GSA, MassHealth, and NH Medicaid exclusion lists; updated annually.
• Social Security Number Trace: Must be run with criminal check covering last 7 years.
• Education Verification: Highest level of education verified through background check.

Documentation Required for Submission

• Resume including past 7 years of work experience with city and state.
• Two recent references (minimum one supervisor or above in skillset).
• Skills checklist specific to the role.
• Candidate submission coversheet (current version).

Additional Notes

• Confirmation of Assignment/POI Form required for all initial assignments and extensions.
• Positive TB skin test requires documentation of a negative chest x-ray after the positive test.
• TB symptom survey required if positive PPD, updated annually.
