Job Opportunity: Physical Therapist Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Summary

Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Patient age range is from birth to over 100 years. Collaborate with patients, families, physicians, and other allied health professionals to identify needs and coordinate services. Maintain thorough and timely electronic documentation and interdisciplinary communication according to agency policies and standards of practice.

Requirements

• License: Must hold a valid Physical Therapist professional license in Massachusetts.
• Experience: Minimum 1 year of Home Care experience required.
• Schedule: Weekend rotations occur once every 6 weeks.
• Travel: Ability to travel to assigned cases is required.

Credentials and Compliance (Selected Key Requirements)

• Professional license verification and primary source verification required.
• MVR (Motor Vehicle Record) check required within 45 days of start.
• Criminal background checks including Massachusetts CORI, national sex offender registry, and exclusion checks required.
• Health requirements include:

  • N95 Fit Test within 1 year of start date, updated annually.
  • Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions.
  • TB test or negative QuantiFERON/T-spot within 3 months of start, updated if >6 month break.
  • Health statement signed by MD/DO/NP/PA within 1 year of start, updated if >6 month break.
  • MMR vaccination or proof of immunity required.
• Submission of a recent professional photo (color, plain background, no glasses).
• Submission of 2 recent references (minimum 1 supervisor or above) in the skillset.
• Submission of a skills checklist and candidate submission coversheet required.
• Education verification and resume covering past 7 years of work experience required.

Additional Notes

• MA Conditional Licenses are not accepted.
• No religious or medical exemptions for flu vaccine or fit testing.
• Fit testing cannot be done onsite; must be completed prior to start.
• All background checks and credential verifications must be current and compliant with agency policies.

This position is ideal for a licensed Physical Therapist with home care experience who is comfortable traveling to patient homes and working a flexible schedule including occasional weekends.
