Description
Physical Therapist - Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 17 weeks
Job Summary
Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Patient ages range from birth to over 100 years. Collaborate with patients, families, physicians, and other allied health professionals to identify needs and coordinate care. Maintain thorough and timely electronic documentation and interdisciplinary communication according to agency policies and standards of practice. Weekend rotations occur once every 6 weeks. Ability to travel to assigned cases is required.
Required Qualifications
- Valid Physical Therapist license in Massachusetts
- Minimum 1 year of Physical Therapist experience within the last year
- Experience in a Home Care setting
- Must be able to travel to patient homes as assigned
- Weekend rotations once every 6 weeks
Onboarding & Compliance Requirements
- Submission of a recent professional color photo (no selfies, plain background)
- Motor Vehicle Record (MVR) check within 45 days of start
- Documentation of immunizations or immunity for MMR (measles, mumps, rubella)
- TB test documentation within 3 months of start date (skin test or blood test)
- Physician-signed health statement within 1 year of start date
- Criminal background check, including Massachusetts CORI and national sex offender registry checks, within 45 days of start
- Education verification of highest degree achieved
- Respirator fit test (N95) within 1 year of start and updated annually (no exemptions allowed)
- Nurse Aide Registry check within 30 days of start (required by MA Department of Public Health for all employees)
- Various attestations and forms related to assignment confirmation and compliance

- Prior travel experience is not explicitly required.
- All documentation and background checks must be current and updated if there is a break in service longer than 6 months.
- Fit testing cannot be performed onsite; must be completed prior to start.
- Weekend rotations are infrequent (once every 6 weeks).
This position is ideal for a licensed Physical Therapist with home care experience who is comfortable providing care in patient residences and collaborating with interdisciplinary teams.
