Description
Job Opportunity: Physical Therapist - Outpatient
Location: Nantucket, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 15 weeks
Job Description
- Outpatient setting only
- Work schedule: 4 shifts per week, each 10 hours long
- Shifts start between 8:00 and 8:30 AM
- Shared housing provided at no cost
Required Qualifications
- Valid Physical Therapist license in Massachusetts
- Minimum of 1 year of Physical Therapist experience, including experience within the last year
- Basic Life Support (BLS) certification issued by the American Heart Association or Red Cross
Required Credentials and Compliance
- Professional license verification and primary source verification required (no conditional licenses accepted)
- Completion of FEMA ICS 100 Training prior to onboarding
- Criminal background check, including Massachusetts CORI and national sex offender registry checks, within 45 days prior to start
- Education verification of highest degree achieved
- Submission of a detailed resume covering the past 7 years of work experience
- Two professional references within the last 4 years (minimum one supervisor-level)
- Skills checklist completed within 1 year of submission
- Health and immunization requirements including:
- TB test within 3 months of start date (or negative QuantiFERON/T-spot blood test)
- MMR vaccination or proof of immunity
- Physician health statement within 1 year of start date
- N95 Fit Test within 1 year of start date, updated annually (no exemptions allowed)
- Criminal and exclusion checks including OIG, GSA, MassHealth, and NH Medicaid lists
- Background checks and credential verifications must be updated if there is a break in assignment greater than 6 months
Additional Information
- Housing agreement form required if staying in provided housing, with updates required if there is a break of 35 days or more
- Some optional but strongly preferred immunizations include Hepatitis B and Tdap vaccines
- Positive TB test requires a negative chest X-ray report
- No onsite fit testing available; must be completed prior to start or at designated locations if assignment is extended
