Job Description

• Position as a Speech Language Pathologist in an outpatient setting.
• Shared housing is available (no guests or pets allowed). Must have own transportation.
• Candidate must have received the flu vaccine.

Required Qualifications

• Valid Speech Language Pathologist license in Massachusetts
• Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross

Additional Information

• Positive TB test requires documentation of a negative chest x-ray after the positive test.
• Name change documentation required if legal name differs from credential or profile documentation.
• TB symptom survey required if positive TB test, updated annually.
• Candidate submission coversheet required for initial assignment submissions.
