Job Opportunity: RN CVOR

Location: Mishawaka, Indiana
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule

• Shift: 7:00 AM – 5:30 PM
• On-Call: 2 nights per week and 2 weekends per month

Required Certifications & Licenses

• BLS (Basic Life Support) – American Heart Association certification required
• ACLS (Advanced Cardiovascular Life Support) – Required for RNs in critical care areas
• NRP (Neonatal Resuscitation Program) – Required if working in Birth Center
• License/Certificate Verification – Must be current within 30 days of start date

Required Health & Safety Compliance

• Fit Test
• Annual TB Questionnaire
• Trinity 10 Panel Drug Screen (must be completed within 30 days of start)
• COVID-19 Vaccine (including booster by Sept 2025, or approved exemption)
• MMR (Measles, Mumps, Rubella) – Proof of 2 vaccinations or IgG titer
• Hepatitis B vaccination series (at least first dose prior to start)
• Influenza Vaccine (or declination/exemption for current flu season)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – with chest x-ray if positive TB as per state requirements
• Varicella – Proof of vaccination or IgG titer
• Physical/Health Statement – Documentation of fitness to work within last year
• Tdap vaccine (or declination if no pediatric interaction)

• Trinity Code of Conduct Form
• Trinity I9 form
• Confidentiality and System Security Agreement
• Integrity and Audit Services Form
• Joint Commission CORE Exam (within last year)
• Conflict of Interest Disclosure Form
• Criminal Background Check (Federal, Nationwide, County for last 7 years)
• Education Verification (highest level of academic education)
• EPLS (Excluded Parties List System) check
• GSA/OIG Exclusion check
• NSO (National Sex Offender) check
• System Access Form (to be completed one week before start)
• OFAC (Office of Foreign Assets Control) check
