Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule

• One 5:30 PM to 6:30 AM call shift per week
• Every 5th weekend on call

Required Credentials & Onboarding Requirements

Licenses & Certifications

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide front and back images of the card.
• License/Certificate Verification: Must be dated within 30 days of start. Online verification required (state verification preferred, e.g., NURSYS for RNs).
• NIHSS Certification
• Trinity Specialty Specific Competency Testing: Specialty exam must be completed within the last year.
• Trinity Skills Checklist: Provided by agency for clinical contractors.

Health & Immunizations

• Fit Test
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include specific substances; no THC allowed.
• COVID-19 Vaccine: Proof of booster by September 2025, or declination/exemption with agency letterhead (declination not accepted in CA or IL). Valid through October 2026.
• MMR: Proof of 2 vaccinations with dates or IgG titer lab report.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead. Flu season dates apply. Declination not accepted in CA or IL.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): TB testing with chest x-ray if positive (within last year for some states, within 5 years for others).
• Varicella: Proof of 1 vaccination or IgG titer lab report.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Tdap Vaccine: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.

• Trinity Code of Conduct Form (signed)
• Trinity I9 Form (completed by agency)
• Confidentiality & Information System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non Employee Upload Form (pages 1-3 and 28 completed and uploaded)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check: Federal, nationwide, county checks for last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification: Highest academic degree or diploma/GED verified by 3rd party screening.
• EPLS (Excluded Parties List System) Check: Dated within 30 days of start.
• GSA/OIG Exclusion Check: Dated within 30 days of start.
• Joint Commission CORE Exam: Completed within last year.
• NSO (National Sex Offender) Check: Dated within 30 days of start, including all aliases and locations.
• System Access Form: Completed and uploaded one week before start.
• Child and Adult Dependent Abuse Training: Required for Iowa assignments.
• Iowa Sing Report: Criminal history, sex offender, dependent adult & child abuse registries check required within 30 days prior to start.
• OFAC Sanctions Check: Completed within 30 days of start.

Submission Requirements

• Trinity Profile: Resume, skills checklist (within last year), and two professional references required for submission.
