Radiology Technician - Indiana (Mishawaka)

Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Schedule:

• 40 hours per week
• 8-hour shifts, 7:30 AM to 4:00 PM
• Monday to Friday, day shift
• On-call rotation

Required Certifications and Licenses:

• BLS (American Heart Association certification required for all direct patient care personnel)
• ARRT certification

Required Credentials and Compliance for Onboarding:

• License/Certificate Verification (dated within 30 days of start)
• Fit Test
• Annual TB Questionnaire
• Trinity 10 Panel Drug Screen (completed within 30 days of start, no THC)
• Trinity Code of Conduct Form (signed)
• Trinity I9 form
• Confidentiality and System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Joint Commission CORE Exam (within the last year)
• Color Vision test
• Latex Allergy Questionnaire
• COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption except for CA and IL)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B vaccination series (at least first shot prior to start)
• Influenza Vaccine (proof or declination for 2024-2025 and 2025-2026 seasons)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (per state guidelines)
• Varicella (proof of vaccination or IgG titer)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Tdap vaccine (current, non-expired; declination accepted if no pediatric interaction)

Professional Requirements:

• Resume, Skills Checklist (within last year), and 2 Professional References required for submission
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (federal, nationwide, county for last 7 years, including SSN verification, dated within 30 days)
• Education Verification (highest academic degree or diploma/GED verified)
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG exclusion check dated within 30 days
• NSO (National Sex Offender) check dated within 30 days
• System Access Form (completed and uploaded one week before start)
• Skills Checklist provided by agency for clinical contractors
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days
• Orientation Guide completion

This position requires thorough credentialing and compliance with health and safety standards, including multiple vaccinations, drug screening, and background checks. The role involves day shifts with on-call rotation in a hospital setting.
