Job Description Highlights

• Every Other Week (EOW) / Every Other Holiday (EOH) Return to Office (RTO) schedule
• Blackout dates for RTO: Mid November to Mid January (submissions during this time may be denied if RTO conflicts)
• Every other holiday rotation required
• Weekend makeup shifts may be required for call-ins or RTO that overlaps scheduled weekends

Required Credentials & Onboarding Requirements

Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide front and back images of the card.
• License/Certificate Verification: Must be dated within 30 days of start. Online verification required (state verification preferred, e.g., NURSYS for RNs/LPNs).
• NIHSS Certification
• Trinity Specialty Specific Competency Testing: Specialty exam completed within the last year.
• Trinity Joint Commission Testing: CORE exam within the last year.

Personal Health & Immunizations

• Fit Test
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include 10 specific substances (no THC).
• COVID-19 Vaccine: Proof of booster by September 2025, or declination/exemption with agency letterhead (declination not accepted in CA or IL).
• MMR: Proof of 2 vaccinations or IgG titer lab report.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead. Declination not accepted in CA or IL. Valid for flu season 10/1/2024 - 4/30/2025.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): TB testing required; positive TB requires chest x-ray within last year (varies by state).
• Varicella: Proof of 1 vaccination or IgG titer lab report.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Tdap Vaccine: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.

Professional & Compliance Documents

• Trinity Code of Conduct Form (signed)
• Trinity I9 Form (completed by agency)
• Confidentiality & System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non Employee Upload Form (pages 1-3 and 28 completed and uploaded)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check: Federal, nationwide, county checks for last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification: Highest academic degree or diploma/GED verified by 3rd party screening.
• EPLS (Excluded Parties List System) Check: Dated within 30 days of start.
• OIG (Office of Inspector General) Check: Dated within 30 days of start.
• NSO (National Sex Offender) Check: Dated within 30 days of start, including all aliases and locations.
• System Access Form: Must be completed and uploaded one week before start.
• Skills Checklist: Provided by agency for clinical contractors.
• Child and Adult Dependent Abuse Training: Required for Iowa assignments.
• Iowa Sing Report: Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required within 30 days prior to start.
• OFAC (Office of Foreign Assets Control) Sanctions Check: Must be completed within 30 days of start.
