Location: Westerville, Ohio
Employment Type: Long-Term Assignment (LTA)
Shift: Evening Shift, 5 days a week, 2:00 PM – 10:30 PM with rotating weekends
Contract Length: 13 weeks

Job Description

• Position involves working as a Medical Laboratory Technician on an evening shift schedule.
• Requires availability for rotating weekends.

Required Credentials & Onboarding Requirements

Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide front and back images of the card.
• License/Certificate Verification: Must be dated within 30 days of start. Online verification page required.

Health & Safety

• Fit Test
• 2 Step PPD OR IGRA (Quantiferon/Tspot)
• MMR: Proof of 2 vaccinations or IgG titer lab report required.
• Hepatitis B: Must have started vaccination series prior to start; proof of vaccination required. Titers not accepted.
• Varicella: Proof of 1 vaccination or IgG titer lab report required.
• Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• COVID-19 Vaccine: Proof of vaccine or booster, declination or exemption with agency letterhead required. Declination not accepted in CA or IL.
• Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead required. Declination not accepted in CA or IL. Flu season dates apply.
• Trinity 10 Panel Drug Screen: Completed within 30 days of start. Includes testing for methadone, phencyclidine, benzodiazepines, cocaine, propoxyphene, amphetamines, opiates, 6-acetyl morphine, methaqualone, barbiturates.

• Specialty Specific Competency Testing: Must be completed within the last year.
• Skills Checklist: Provided by agency for clinical contractors.
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check: Federal, nationwide, county checks for last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification: Highest level of academic education verified by third party screening.
• EPLS (Excluded Parties List System) Check: Dated within 30 days of start.
• GSA/OIG Exclusion Check: Dated within 30 days of start.
• NSO (National Sex Offender) Check: Dated within 30 days of start.
• System Access Form: Must be completed and uploaded one week before start.
• Joint Commission CORE Exam: Completed within the last year.
