Job Opportunity: Medical Laboratory Technician (MLT)

Location: Oelwein, Iowa
Shift: 3rd shift, 11:00 PM – 7:30 AM (Weekdays)
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Required Credentials & Certifications

• Certification: ASCP or AMT required
• BLS: American Heart Association BLS certification required for all direct patient care personnel
• License/Certificate Verification: Must be dated within 30 days of start
• Fit Test: Required
• Drug Screen: Trinity 10 Panel Drug Screen completed within 30 days of start (includes methadone, phencyclidine, benzodiazepines, cocaine, propoxyphene, amphetamines, opiates, 6-acetyl morphine, methaqualone, barbiturates; NO THC)
• COVID-19 Vaccine: Proof of booster by Sept 2025, declination or exemption accepted except in CA or IL
• Influenza Vaccine: Proof of vaccine, declination, or exemption required for flu season 10/1/2024 - 4/30/2025 (declination not accepted in CA or IL)
• MMR: Proof of 2 vaccinations or IgG titer lab report
• Hepatitis B: Must have started vaccination series prior to start (3 shots over 6 months); titers not accepted
• Varicella: Proof of 1 vaccination or IgG titer lab report
• Tdap: Current, non-expired vaccine required; declination accepted if no pediatric interaction
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; positive TB requires chest x-ray within last year (specific states) or within 5 years elsewhere
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work

Professional & Compliance Requirements

• Trinity Code of Conduct Form: Signed and on file
• Trinity I9: Completed by agency
• Confidentiality & Security Agreement: Signed and on file
• Integrity and Audit Services Form: Signed shortly before start
• Non Employee Upload Form: Completed and uploaded (pages 1-3 and 28)
• Specialty Specific Competency Testing: Completed within last year, copy attached
• Conflict of Interest Disclosure Form: Signed and on file
• Criminal Background Check: Federal, nationwide, county checks for last 7 years; SSN verification; dated within 30 days of start
• Education Verification: Highest academic degree or diploma/GED verified
• EPLS (Excluded Parties List System): Report dated within 30 days of start
• GSA/OIG Exclusion Check: Report dated within 30 days of start
• Joint Commission CORE Exam: Completed within last year
• NSO (National Sex Offender) Check: Dated within 30 days of start, all aliases checked
• System Access Form: Completed and uploaded one week before start
• Skills Checklist: Provided by agency for clinical contractors
• Child and Adult Dependent Abuse Training: Required for Iowa assignments
• Iowa Sing Report: Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required within 30 days prior to start
• OFAC Sanctions Check: Completed within 30 days of start
