Position Details

Required Experience

Health & Immunization Requirements

• Fit Test
• Trinity 10 Panel Drug Screen (9 or 10 panel drug screen completed within 30 days of start, no THC)
• COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption except for CA or IL)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B (must have started vaccination series prior to start; titers not accepted)
• Influenza Vaccine (proof or declination/exemption; flu season dates apply)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Varicella (proof of vaccination or IgG titer)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Tdap (current vaccine; declination accepted if no pediatric interaction)

Professional & Compliance Requirements

• Trinity Profile (resume, skills checklist within a year, 2 professional references)
• Trinity Code of Conduct Form
• Trinity I9 Verification
• Confidentiality & System Security Agreement
• Integrity and Audit Services Form (signed shortly before start)

This position requires a well-prepared ICU RN with all certifications, immunizations, and compliance documentation up to date and verifiable within specified timeframes. The assignment is for approximately 13 weeks with a schedule alternating every other week and holiday.
