Physical Therapist (PT) Job Opportunity

Location: Baker City, Oregon
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Schedule:

• 40 hours per week
• Dayshift: 7:30 AM – 6:00 PM
• Occasional Saturday coverage

Facility Overview

• Critical access rural hospital
• Large portion of inpatient and swing bed rehabilitation
• Outpatient primarily orthopedics, with potential neuro cases depending on experience

Required Certifications and Licenses

• BLS (American Heart Association certification required for all direct patient care personnel)
• Current state license verification (dated within 30 days of start)

Health and Safety Requirements

• Fit Test
• Trinity 10 Panel Drug Screen (9 or 10 panel drug screen completed within 30 days of start; no THC)
• COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption where allowed)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B vaccination series (proof of at least first vaccination prior to start)
• Influenza Vaccine (proof or declination for current flu season)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Varicella (proof of vaccination or IgG titer)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Color Vision test

• Trinity Code of Conduct Form
• Trinity I9 form
• Confidentiality and System Security Agreement
• Integrity and Audit Services Form
• Motor Vehicle Report (MVR) dated within 30 days of start

• Criminal Background Check (federal, nationwide, county for last 7 years, including SSN verification)
• Education Verification (highest academic degree or diploma/GED)
• EPLS (Excluded Parties List System) check within 30 days
