Required Health & Immunizations

• Fit Test
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days), must include specific substances; no THC allowed
• COVID-19 Vaccine: Proof of September 2025 booster, declination or exemption accepted (except for CA or IL)
• MMR: Proof of 2 vaccinations or IgG titer lab report
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted
• Influenza Vaccine: Proof of vaccine, declination, or exemption for 2024-2025 season (declination not accepted for CA or IL)
• 2 Step PPD OR IGRA (Quantiferon/Tspot): TB testing with chest x-ray if positive (within last year for some states, within 5 years for others)
• Varicella: Proof of 1 vaccination or IgG titer lab report
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work
• Tdap Vaccine: Current, non-expired; declinations accepted if no pediatric interaction
