Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)

Required Certifications & Licenses

• BLS (American Heart Association)
• PALS (or Emergency Nurse Pediatric Course - ENPC)
• ACLS (American Heart Association)
• TNCC (Trauma Nursing Core Course)

• Valid nursing license with verification dated within 30 days of start

Required Health & Immunization Documentation

• Proof of COVID-19 vaccination or approved declination/exemption (booster required by Sept 2025; declination not accepted in CA or IL)
• Influenza vaccine or approved declination/exemption (valid for current flu season)
• MMR (2 vaccinations or IgG titer lab report)
• Hepatitis B vaccination series (1st shot prior to start, 2nd shot 30 days later, 3rd shot 6 months later; titers not accepted)
• Varicella vaccination or IgG titer lab report
• Tdap vaccine (current, non-expired; declination accepted if no pediatric interaction)
• 2 Step PPD or IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (per regional requirements)
• Physical/Health Statement confirming fitness to work (exam within last year)
• Fit Test (specific to Iowa, 1860 mask only)
• 10 Panel Drug Screen completed within 30 days of start (includes methadone, phencyclidine, benzodiazepines, cocaine, propoxyphene, amphetamines, opiates, 6-acetyl morphine, methaqualone, barbiturates)

Competency & Training Requirements

• Specialty Specific Competency Testing (exam within last year)
• Trinity Skills Checklist (provided by agency for clinical contractors)
• Joint Commission CORE Exam (within last year)
• Child and Adult Dependent Abuse Training (Iowa only)

• License and certification verifications must be uploaded with machine-generated date within 30 days of start
