Description
Job Opportunity: Nuclear Medicine Technician
Location: Plymouth, Indiana
Employment Type: Long-Term Assignment (approximately 13 weeks)
Work Schedule: Primarily Monday through Friday at one location, with occasional assistance at a nearby campus as needed
Required Qualifications
- Valid Nuclear Medicine Technologist license in Indiana
- Minimum of 1 year of experience as a Nuclear Medicine Technologist within the last year
Onboarding and Compliance Requirements
Candidates must provide the following documentation, all current and valid as specified:
Certifications and Licenses
- BLS certification (front and back images)
- License/certification verification dated within 30 days of start
Health and Immunizations
- Fit Test
- Annual TB Questionnaire
- 10-panel drug screen completed within 30 days of start (specific substances tested, no THC)
- COVID-19 vaccine proof including booster or approved declination/exemption (with agency letterhead)
- MMR: Proof of 2 vaccinations or IgG titer lab report
- Hepatitis B: At least first vaccination in series prior to start
- Influenza vaccine or declination for current flu season
- 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
- Varicella: Proof of vaccination or IgG titer lab report
- Physical/Health Statement confirming fitness to work within the last year
- Tdap vaccine or declination if no pediatric interaction
Professional and Background Checks
- Signed Code of Conduct form
- Completed I9 form
- Confidentiality and System Security Agreement
- Integrity and Audit Services Form signed shortly before start
- Joint Commission CORE Exam within the last year
- Conflict of Interest Disclosure Form
- Criminal Background Check (Federal, Nationwide, County for last 7 years) including SSN verification, dated within 30 days of start
- Education Verification (highest academic degree or equivalent)
- EPLS (Excluded Parties List System) report dated within 30 days
- OIG (Office of Inspector General) exclusion report dated within 30 days
- NSO (National Sex Offender) check dated within 30 days
- OFAC (Office of Foreign Assets Control) sanctions check dated within 30 days
- System Access Form completed and uploaded one week before start
- Skills Checklist provided by agency for clinical contractors

- Resume, skills checklist (within one year), and two professional references are required for submission.
