Description
Radiation Therapist Position - Idaho, Nampa
Job Details
- Employment Type: Long-Term Assignment (LTA)
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM
- Hours: 36-40 hours per week
- No call or holiday shifts required
- Contract Length: 13 weeks
- High possibility of extension after initial assignment
Required Qualifications
- Valid Radiation Therapist license for the state of Idaho, verified within 30 days of start
- Minimum of 1 year of Radiation Therapist experience, including experience within the last year
- American Heart Association BLS certification
- Completion of a skills checklist provided by the agency (clinical contractors only)
- Fit to work documentation from a physical exam within the last year
- Current immunizations and health screenings including:
- COVID-19 vaccine (including booster or approved exemption)
- Influenza vaccine or approved exemption (valid for upcoming flu season)
- MMR (proof of 2 vaccinations or IgG titer)
- Hepatitis B vaccination series (at least first dose prior to start)
- Varicella vaccination or IgG titer proof
- Tdap vaccine (declination accepted if no pediatric interaction)
- 2-step PPD or IGRA TB test (with chest x-ray if positive, per regional requirements)
- Negative 10-panel drug screen completed within 30 days of start (no THC)
- Color vision test
- Fit test (respirator)
Background and Compliance Requirements
- Federal, nationwide, and county criminal background check covering the last 7 years, including SSN verification, dated within 30 days of start
- Motor Vehicle Report (MVR) dated within 30 days of start, showing valid license
- Education verification of highest academic degree or diploma/GED
- Verification of license/certification online with machine-generated date within 30 days of start
- Completion of various professional forms and agreements including:
- Code of Conduct
- Confidentiality and Information Security Agreement
- Integrity and Audit Services Form
- Conflict of Interest Disclosure
- Criminal Background Check
- EPLS (Excluded Parties List System) check
- GSA/OIG exclusion check
- Joint Commission CORE exam within the last year
- NSO (National Sex Offender) check dated within 30 days of start
- System Access Form completed and uploaded one week before start
- Standard I-9 form completed by the agency
