Ultrasound Tech - Iowa, Waterloo
Job Details
- Employment Type: Long-Term Assignment (LTA)
- Contract Length: 13 weeks
- Shift: 2:00 PM – 10:00 PM
- On-Call Requirement: One night per week on call
Required Qualifications
- Ultrasound Technologist with at least 1 year of experience, including experience within the last year
- Current and valid Ultrasound Technician license or certification as required by the state of Iowa
- BLS certification issued by the American Heart Association
Onboarding and Compliance Requirements
- Current license/certification verification dated within 30 days of start
- Fit test
- 10-panel drug screen completed within 30 days of start (no THC)
- Specialty-specific competency testing completed within the last year
- Color vision test
- COVID-19 vaccine proof (including booster or approved declination/exemption; declination not accepted in CA or IL)
- MMR vaccination proof (2 doses or IgG titer)
- Hepatitis B vaccination series started prior to start date (titers not accepted)
- Influenza vaccine proof or declination (optional)
- TB screening: 2-step PPD or IGRA; if positive, chest x-ray per regional requirements
- Varicella vaccination proof or IgG titer
- Physical/health statement within the last year confirming fitness to work
- TDAP vaccine or declination (declination accepted if no pediatric interaction)
- Background checks including:
- Federal, nationwide, and county criminal checks from last 7 years
- Social Security Number verification
- State-specific criminal and abuse registries (Iowa-specific checks required)
- EPLS, OIG, OFAC sanctions checks within 30 days of start
- NSO (National Sex Offender) check within 30 days of start
- Trinity-specific forms and agreements signed and on file prior to start:
- Code of Conduct
- Confidentiality and Information Security Agreement
- Integrity and Audit Services Form
- Non-Employee Upload Form
- Conflict of Interest Disclosure Form
- System Access Form (completed and uploaded one week before start)
- Education verification (highest academic degree or diploma/GED)
- Resume and skills checklist within the last year
- Professional references (2)
Additional Notes
- Agency will provide the skills checklist for clinical contractors.
- All documentation must be uploaded to the credentialing system prior to or at onboarding as specified.
- This position requires adherence to all health and safety protocols and compliance with state and federal regulations.
