Job Details

• Position: Registered Nurse (Operating Room)
• Location: Ontario, Oregon
• Employment Type: Long-Term Assignment (approximately 13 weeks)
• Shift: 06:45 AM – 5:15 PM

Required Qualifications

• Valid RN license in Oregon
• Minimum of 1 year of Operating Room (OR) experience within the last year
• BLS certification issued by the American Heart Association
• PALS certification
• ACLS certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Required Onboarding Documentation and Compliance

• License and certification verifications dated within 30 days of start.
• Fit test.
• 10-panel drug screen completed within 30 days of start (no THC).
• Specialty-specific competency testing completed within the last year.
• COVID-19 vaccination proof (including booster by September 2025), or approved declination/exemption (declination not accepted in CA or IL).
• MMR vaccination proof (2 doses or IgG titer).
• Hepatitis B vaccination series initiated prior to start.
• Influenza vaccine proof or declination (optional).
• 2-step PPD or IGRA test; if positive, chest x-ray per regional requirements.
• Varicella vaccination proof or IgG titer.
• Physical/health statement confirming fitness to work within the last year.
• Completion of professional forms including:

  • Code of Conduct
  • Conflict of Interest Disclosure
  • Criminal Background Check (7 years, including SSN verification)
  • Education Verification
  • EPLS and OIG exclusion checks (within 30 days)
  • Integrity and Audit Services Form (signed near start date)
  • Joint Commission CORE Exam (within 1 year)
  • National Sex Offender Registry check (within 30 days)
  • System Access Form (completed and uploaded one week before start)
• Resume and skills checklist (within 1 year) and two professional references.

• All documentation must be uploaded to the credentialing system prior to or at onboarding.
• Specialty competency and skills checklists are required for clinical contractors.
• Drug screen panel includes methadone, phencyclidine, benzodiazepines, cocaine, propoxyphene, amphetamines, opiates, 6-acetyl morphine, methaqualone, barbiturates.
