Radiation Therapist Position - Clinton, Iowa

Job Details

• Employment Type: Long-Term Assignment (LTA)
• Contract Length: 13 weeks
• Shift: Monday to Friday, 7:00 a.m. to 3:30 p.m.
• No holiday or weekend hours

Required Qualifications

• Valid Radiation Therapist license in Iowa
• Minimum of 1 year of Radiation Therapist experience, including experience within the last year
• Current BLS certification issued by the American Heart Association

Compliance and Background Checks

• Federal, nationwide, and county criminal background check covering the last 7 years, including SSN verification and alias checks.
• Iowa-specific criminal history, sex offender, dependent adult & child abuse registry checks.
• EPLS (Excluded Parties List System) and OIG (Office of Inspector General) exclusion checks.
• NSO (National Sex Offender) check dated within 30 days of start.
• Conflict of Interest Disclosure form.
• Code of Conduct form.
• Confidentiality and Information System Security Agreement.
• Integrity and Audit Services form.
• Non-Employee Upload form.
• Education verification (highest academic degree or diploma/GED).
• Trinity Profile submission including resume, skills checklist (within a year), and two professional references.
• System Access form completed and uploaded one week before start.
• Joint Commission CORE exam within the last year.

Additional Information

• Agency will provide a skills checklist for clinical contractors.
• All license and certification verifications must be dated within 30 days of start.
• All required documents must be uploaded to the credentialing system prior to onboarding.
