Work Schedule

• Dayshift: 06:30 AM – 05:00 PM
• 40 hours per week, scheduled as 4 days of 10 hours each
• 14 days of on-call within a 6-week rotation
• Every third weekend on-call
• Possible holiday on-call required

Required Credentials and Compliance

Immunizations & Health Requirements

• COVID-19 Vaccine: Proof of September 2024 booster, declination or exemption (declination not accepted for CA or IL)
• MMR: Proof of 2 vaccinations with dates or primary source IgG titer lab report
• Hepatitis B: Must have completed vaccination series (3 shots); titers not accepted
• Influenza Vaccine: Proof of vaccine, declination, or exemption (optional) for flu season 10/1/2024–4/30/2025
• Varicella: Proof of 1 vaccination with date or IgG titer lab report
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Screening within 30 days (PPD) or 90 days (IGRA)
• Fit Test: Required prior to onboarding
• Physical/Health Statement: Documentation from exam within the last year confirming fitness to work
• Trinity Specific COVID Questionnaire: Must be completed prior to start

Drug Screening

• Trinity 9 Panel Drug Screen: Completed within 30 days of start (includes cocaine, amphetamines, opiates, and others; NO THC allowed)

Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel
• License/Certificate Verification: Must be current and verified online within 30 days of start
• Trinity Specialty Specific Competency Testing: Specialty exam completed within the last year

Professional Requirements

• Resume, Skills Checklist, and 2 Professional References
• Trinity Profile: Resume and skills checklist submission required
• Trinity Confidentiality Agreement: Signed agreement regarding patient and proprietary information
• Conflict of Interest Disclosure Form: Signed and on file
• Criminal Background Check: Federal, nationwide, and county checks covering last 7 years, including SSN verification, dated within 30 days of start
• Education Verification: Highest level of academic education verified (degree or diploma/GED)
• Exclusion Lists Checks: EPLS, OIG/GSA, NSO checks within 30 days of start
• Integrity and Audit Services Form: Signed prior to start
• Trinity Code of Conduct Form: Signed and on file
• Trinity I9: Completed by agency
• Trinity System Access Form: Completed and uploaded one week before start

Additional Testing & Checklists

• Trinity Joint Commission Testing: CORE exam within the last year
