Radiology Technician - Trinity Health, MercyOne Waterloo Medical Center

Location: Waterloo, Iowa
Employment Type: Long-Term Assignment (12 months)

Work Schedule

• Monday to Friday, 7:00 AM to 3:30 PM
• Call every third weekend
• Two call days per week during weekdays (except on call weekends)

Required Certifications

• ACLS (Advanced Cardiovascular Life Support)
• BLS (Basic Life Support) - American Heart Association certification required for all direct patient care personnel

• Trinity Child and Adult Dependent Abuse Training (Iowa only)
• Iowa Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check (within 30 days prior to start)
• OFAC Sanctions Search (within 30 days prior to start)
• Trinity Skills Checklist (clinical contractors only)
• License/Certificate Verification (dated within 30 days of start)
• Fit Test
• 9 Panel Drug Screen (within 30 days of start, no THC)
• Trinity Code of Conduct Form
• Trinity I9 Form
• Confidentiality and Information System Security Agreement
• Integrity and Audit Services Form
• Non-Employee Upload Form
• Specialty Specific Competency Testing (exam within past year)
• Color Vision Test
• COVID-19 Vaccine proof, booster, declination, or exemption (declination not accepted in CA or IL)
• MMR Vaccination or IgG titer proof
• Hepatitis B Vaccination series proof (titers not accepted)
• Influenza Vaccine (optional)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) Tuberculosis test
• Varicella Vaccination or IgG titer proof
• Physical/Health Statement (fit to work exam within last year)
• Trinity Profile (Resume, Skills Checklist within a year, 2 Professional References)
• Conflict of Interest Disclosure Form
• Criminal Background Check (Federal, Nationwide, County - last 7 years, within 30 days)
• Education Verification (highest level of academic education)
• EPLS (Excluded Parties List System) Check (within 30 days)
• GSA/OIG Exclusion Check (within 30 days)
• Joint Commission CORE Exam (within past year)
• NSO (National Sex Offender) Check (within 30 days)
• System Access Form (completed and uploaded one week before start)
• Specific COVID Questionnaire

Additional Information

• All documentation must be scanned and uploaded to the Trinity VMS system as required.
• Compliance with all credentialing and background checks is mandatory before onboarding.
• This position requires adherence to Trinity Health policies and procedures, including confidentiality and code of conduct.
