Job Opportunity: RN Medical/Surgical - Long Term Assignment (LTA)

Schedule: Every third weekend and holiday rotation (includes at least 1 holiday during assignment)

Required Credentials and Onboarding Information

Licenses & Certifications

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide scanned front and back copies with expiration date.
• License/Certificate Verification: Must be dated within 30 days of start. Online verification with machine-generated date required. For RNs and LPNs, state verification via NURSYS preferred.
• NIHSS Certification
• Trinity Specialty Specific Competency Testing: Specialty exam must be completed within the last year.
• Trinity Joint Commission CORE Exam: Must be completed within the last year.

Personal Health Requirements

• Fit Test
• Trinity 10 Panel Drug Screen: 9-panel drug screen (no THC) completed and resulted within 30 days of start, preferably within 14 days, with MRO signature.
• COVID-19 Vaccine: Proof of September 2024 booster, declination, or exemption (declination not accepted in CA or IL).
• MMR: Proof of 2 vaccinations with dates or IgG titer lab report.
• Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Titers not accepted.
• Influenza Vaccine: Optional but recommended; proof of vaccine, declination, or exemption accepted (declination not accepted in CA or IL).
• 2 Step PPD OR IGRA (Quantiferon/Tspot): TB testing required; positive TB requires chest x-ray within last year (for some states) or within 5 years for others.
• Varicella: Proof of 1 vaccination or IgG titer lab report.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Tdap Vaccine: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.

Professional & Compliance Requirements

• Trinity Code of Conduct Form: Signed form required.
• Trinity I9: Standard I9 completed by agency.
• Confidentiality & Security Agreement: Signed agreement regarding patient and proprietary information.
• Integrity and Audit Services Form: Signed within days prior to start.
• Non-Employee Upload Form: Completed and uploaded pages 1-3 and 28.
• Conflict of Interest Disclosure Form: Signed form required.
• Criminal Background Check: Federal, nationwide, county checks for last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification: Highest academic education verified by 3rd party screening.
• EPLS (Excluded Parties List System) Check: Scanned report dated within 30 days.
• OIG (Office of Inspector General) Check: Scanned report dated within 30 days.
• NSO (National Sex Offender) Check: Dated within 30 days, all aliases checked.
• System Access Form: Must be completed and uploaded one week before start.
• Skills Checklist: Provided by agency for clinical contractors.
• Child and Adult Dependent Abuse Training: Required for Iowa assignments.
• Iowa Sing Report: Criminal history, sex offender, dependent adult & child abuse registries check required within 30 days prior to start.
• OFAC (Office of Foreign Assets Control) Check: Must be completed within 30 days of start.

• Trinity Profile: Resume, skills checklist (within a year), and 2 professional references required for submission.

This position is ideal for RNs with medical/surgical experience who meet the above credentialing and compliance requirements and are available for a 13-week long-term assignment with holiday and weekend rotations in Dyersville, Iowa.
