Job Opportunity: Physical Therapist (PT)
Location: Monticello, Iowa
Facility: Above & Beyond Home Health and Hospice
Hospital System: Trinity Health
Employment Type: Long-Term Assignment (LTA)
Contract Length: 12 weeks
Schedule:
- 40 hours per week
- Shift options: 8 AM - 4 PM or 9 AM - 5 PM
- Weekends as needed
- Epic experience preferred but not required
Key Requirements and Onboarding Credentials
Certifications & Licenses
- BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel.
- License/Certificate Verification: Must be current within 30 days of start date. State verification required.
Health & Immunizations
- Fit Test: Required prior to start.
- Trinity 10 Panel Drug Screen: Must be completed and resulted before start, including specific substances (no THC).
- COVID-19 Vaccine: Proof of September 2024 booster, declination or exemption accepted (except CA or IL).
- MMR: Proof of 2 vaccinations or IgG titer required.
- Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
- Influenza Vaccine: Optional but recommended; proof or declination accepted (except CA or IL).
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; positive TB requires chest x-ray per regional guidelines.
- Varicella: Proof of vaccination or IgG titer required.
- Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
- Tdap: Current, non-expired vaccine required; declination accepted if no pediatric interaction.
Professional & Compliance Documents
- Trinity Code of Conduct Form: Signed form required.
- Trinity I9: Standard employment eligibility verification completed by agency.
- Confidentiality & Security Agreement: Signed agreement on patient and proprietary information required.
- Integrity and Audit Services Form: Signed shortly before start date.
- Non-Employee Upload Form: Completed and uploaded.
- Specialty Specific Competency Testing: Specialty exam completed within the last year required.
- Conflict of Interest Disclosure Form: Signed form required.
- Criminal Background Check: Federal, nationwide, and county checks covering last 7 years, including SSN verification, dated within 30 days of start.
- Education Verification: Highest academic degree or diploma verified by third party screening.
- EPLS (Excluded Parties List System): Report dated within 30 days required.
- GSA/OIG Exclusion Check: Report dated within 30 days required.
- Joint Commission CORE Exam: Completed within the last year.
- NSO (National Sex Offender) Check: Dated within 30 days of start, including all aliases.
- System Access Form: Must be completed and uploaded one week before start.
- Skills Checklist: Provided by agency for clinical contractors.
- Child and Adult Dependent Abuse Training: Required for Iowa assignments.
- Iowa Sing Report: Criminal history, sex offender, and abuse registry checks required within 30 days prior to start.
- OFAC Sanctions Check: Must be completed within 30 days of start.
This position requires thorough credentialing and compliance documentation prior to onboarding. Candidates should be prepared to provide all required health, professional, and legal verifications as outlined.
