Required Certifications & Training

• BLS (Basic Life Support) certification required (American Heart Association)
• Trinity Child and Adult Dependent Abuse Training (Iowa only)
• Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check (Iowa law requirement)
• OFAC Sanctions Screening (must be completed within 30 days of start)
• Skills Checklist (provided by agency for clinical contractors)
• Genesis Medical Center PowerPoint training (signature page required)
• License/Certificate Verification (must be dated within 30 days of start)
• Fit Test
• 9 Panel Drug Screen (completed within 30 days of start, no THC)
• Code of Conduct Form

• Confidentiality and Information Security Agreement
• Integrity and Audit Services Form
• Non-Employee Upload Form
• Specialty Specific Competency Testing (attach specialty exam completed within a year)
• Color Vision Test
• COVID-19 Vaccine proof or exemption (with agency letterhead)
• MMR vaccination proof or immunity
• Hepatitis B vaccination series (titers not accepted)
• Influenza Vaccine (optional)
• 2 Step PPD or IGRA (Quantiferon/Tspot) test
• Varicella vaccination proof or immunity
• Physical/Health Statement (fit to work exam within last year)
• Trinity Profile (resume, skills checklist, 2 professional references)
• Conflict of Interest Disclosure Form
• Criminal Background Check (Federal, Nationwide, County for last 7 years, SSN verification)
• Education Verification (highest level of academic education)
• EPLS (Excluded Parties List System) screening
• GSA/OIG Exclusion List screening
• Joint Commission CORE Exam (within a year)
• NSO (National Sex Offender) check (within 30 days of start)
• System Access Form (must be completed and uploaded one week before start)
• Specific COVID Questionnaire
