Key Requirements and Credentials

Licensure & Certifications

• Registered Nurse (RN) license valid in Iowa with online verification dated within 30 days of start
• BLS (Basic Life Support) certification from American Heart Association (front and back of card required)
• ACLS (Advanced Cardiovascular Life Support) certification for RNs in critical care areas (CCU, ED, PACU, MICU, SICU)
• NIHSS (National Institutes of Health Stroke Scale) certification
• Trinity Specialty Specific Competency Testing (specialty exam within the last year)
• Trinity Joint Commission Testing (CORE Exam) within the last year

Background & Compliance Checks

• Iowa-specific Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check (must be done within 30 days prior to start)
• Trinity 9 Panel Drug Screen completed within 30 days of start (no THC)
• Trinity Criminal Background Check covering last 7 years, including SSN verification and employment verification
• Trinity EPLS (Excluded Parties List System) and Trinity GSA/OIG checks dated within past 30 days
• Trinity NSO (National Sex Offender) Check dated within 30 days of start
• Trinity IA Sing Report (Iowa-specific screening)
• Trinity OFAC (Office of Foreign Assets Control) screening within 30 days of start

Health & Immunizations

• Fit Test (respirator fit testing)
• COVID-19 Vaccine proof of booster or approved exemption (declination not accepted in CA or IL)
• MMR (Measles, Mumps, Rubella) vaccination or immunity proof
• Hepatitis B vaccination series completed or proof (titers not accepted)
• Influenza Vaccine (optional but recommended) for the 2024-2025 season
• 2 Step PPD OR IGRA (Quantiferon/Tspot) tuberculosis screening
• Varicella vaccination or immunity proof
• Physical/Health Statement confirming fitness to work, dated within the last year

Professional & Administrative

• Completion of Trinity Child and Adult Dependent Abuse Training
• Signed Trinity Code of Conduct Form
• Signed Trinity Confidentiality of Patient & Proprietary Information and System Security Agreement
• Signed Trinity Integrity and Audit Services Form
• Completion of Trinity Non Employee Upload Form (pages 1-3 and 28)
• Submission of Trinity Profile including resume, skills checklist (within a year), and 2 professional references
• Completion of Trinity Conflict of Interest Disclosure Form
• Completion of Trinity I9 form
• Completion of Trinity System Access Form (one week before start)
• Completion of Trinity Specific Covid Questionnaire

Additional Notes

• All required documents and certifications must be uploaded to Trinity VMS prior to or at onboarding.
• Drug screen and background checks must be current and compliant with Trinity Health policies.
• This position is Iowa-only and requires adherence to state-specific credentialing and compliance.
