Job Opportunity: Physical Therapist (PT) - Travel Position

Job Requirements and Details

• Must hold a valid Physical Therapist (PT) license.
• Basic Life Support (BLS) certification required.
• Work schedule includes 6 weekend days during the 13-week contract; no holiday shifts.
• Minimum 2 years of acute or inpatient experience required.
• Experience with EPIC electronic health record system preferred.
• May be required to float onsite within skill set or to other sites within 40 miles as needed.

Required Credentials and Compliance

Certifications and Exams

• Ascension Specialty Exam: Must be completed prior to first assignment and updated every 3 years. Minimum 80% passing score required.
• Basic Life Support (BLS): Current certification required.

Vaccinations and Health Requirements

• MMR: Documented proof of two MMR vaccines or positive IGG titer. No declinations accepted. If negative titer, two doses of MMR vaccine four weeks apart required.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Varicella: Proof of two vaccines, serological immunity, or verified history. No declinations accepted.
• Tdap: Valid within 10 years. No declinations accepted.
• Influenza Vaccine: Recommended during flu season (Oct 1 - Mar 31). Exemptions accepted but mask required.
• Fit Test: Required for all travelers; preferred before start date. Specific respirator models required (3M 1860 Regular, 3M 1860 Small, or 3M 1870+).
• Tuberculosis Screening: 2-step PPD or IGRA test within required timeframes. If positive, chest X-ray and annual TB questionnaire required.

Health and Physical Requirements

• Health Statement: Statement of good health or completed physical exam prior to assignment, updated annually.
• Drug Screen: 12-panel drug screen within 30 days prior to start, updated annually.
• Positive PPD: Negative chest X-ray and annual TB questionnaire if applicable.

Professional Documentation

• Driver’s License/State ID: Must be current and match address on file.
• Resume/Profile: Required for submission.
• Professional References: At least two qualifying references prior to assignment.
• Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender, and other checks within 30 days of assignment.
• Employment Verification: Third-party verification for the past seven years.
• Education Verification: Verification of relevant education within 90 days of start.
• Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.
• Badge Photo: Forward-facing headshot with white or blue background, no alterations.
• Confirmation of Assignment: Provided upon initial assignment and updated with extensions.

Additional Notes

• Flu vaccine exemptions require mask-wearing at all times during flu season.
• Travel assignments may require flexibility to float to other nearby sites.
• All credentials and verifications must be maintained and updated as required throughout the contract.
