Description

Job Opportunity: Physical Therapist - Home Health (Travel/Contract)

Location:
Agawam, Massachusetts (Northeast MA/CT POD Home Health)
Territory covers: Hampden County, Hampshire County, and Franklin County
Must be willing to travel up to 75 miles from home clinic (approx. 200-250 miles/week)

Job Description

  • Provide physical therapy services in a home health setting with patient home visits.
  • Average caseload: 5-6 patients per day.
  • Types of visits: Start of Care, Follow Ups.
  • Schedule: Monday to Friday, 8 AM to 5 PM.
  • Weekend rotation: Every 6th weekend.
  • No on-call required.
  • Productivity expectation: Approximately 30 points per week (points assigned based on visit type and activities such as documentation and travel).
  • Use of EMR system: HomeCare HomeBase.
  • Must have reliable transportation, current driver’s license, and agency-required liability insurance.
  • Some positions may have a Baylor Shift schedule (4x10s, Friday-Monday), but most are standard 5x8s Monday-Friday.

Requirements

  • Experience: Minimum 1 full year of recent adult home health physical therapy experience. First-timers without home health experience are not accepted.
  • Certifications: Current CPR and BLS certification required.
  • Licensing: Must have a valid license in hand (pending licenses not accepted unless only a few days away from issuance).
  • Local candidates: Accepted at the same rate.
  • Mileage: Reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).
  • On-call: Varies by location; discuss during interview.
  • Weekend requirements: Varies; discuss during interview.

Compliance and Documentation Requirements

Candidates must complete and maintain the following prior to and during assignment:

  • Valid Driver’s License/State ID.
  • Liability and personal/auto insurance (policy must be current and under candidate’s name or list candidate as a driver).
  • Background checks including:
    • 7-year standard background check (SS trace, county, nationwide, sex offender, state-specific).
    • State-specific clearances as required.
  • Health documents:
    • Physical exam within 12 months confirming good health and no communicable diseases.
    • Negative TB test or chest X-ray within 12 months.
    • Proof of immunizations or titers for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella.
    • Drug screening (10-panel).
    • Seasonal flu vaccination or declination.
    • COVID-19 vaccination series or facility-approved declination (Massachusetts requires full vaccination with no declinations).
  • Compliance training and policy acknowledgments including:
    • HIPAA and privacy policies.
    • Code of conduct.
    • Confidentiality agreements.
    • Home health and hospice-specific policy reviews and quizzes.
    • COVID-19 policy and attestation.
  • Education and work history verification.
  • References (two clinical references required).
  • Fingerprinting as required by state and facility.

Additional Notes

  • Candidates must sign new supplier packets at the start of each assignment and after any break in service.
  • Annual renewals of compliance documents are required if working more than one year consecutively.
  • Some positions may require additional state-specific forms or attestations.
  • Candidates should be prepared to discuss weekend and on-call requirements during the interview.
