• Influenza Vaccine (Optional, but recommended for 10/1/2024 - 4/30/2025 season)
• 2 Step PPD or IGRA (Quantiferon/Tspot) (Required)
• Varicella Vaccination or IgG Titer Proof (One vaccination or lab print-out required)
• Physical/Health Statement (Documentation from exam within last year confirming fitness to work)
• Trinity Profile (Resume, skills checklist within a year, and 2 professional references required for submission)
• Conflict of Interest Disclosure Form (Signed form required)
• Criminal Background Check (Federal, nationwide, county for last 7 years; SSN verification; dated within 30 days of start)
• Education Verification (Highest academic education verified; certifications required if applicable)
• Excluded Parties List System (EPLS) Check (Must be dated within past 30 days)
• OIG/GSA Exclusion Check (Must be dated within past 30 days)
• Joint Commission CORE Exam (Completed within a year)
