Location: Bartlesville, Oklahoma
Facility: OKTUL Jane Phillips Medical Center
Hospital System: Ascension Health
Contract Length: 13 weeks
Job Description
- Position: Physical Therapist (Occupational Therapist role)
- Employment Type: Travel
Required Credentials and Compliance
Immunizations & Vaccines
- MMR: Documented proof of two MMR vaccines or positive IGG titer required. No declinations accepted. If negative titer, two doses of MMR vaccine must be administered four weeks apart.
- Hepatitis B: Documentation of vaccination series completion or signed declination required.
- Varicella: Proof of two Varicella vaccines, serological immunity, or verified history required. No declinations accepted.
- Tdap: Valid within 10 years from administration date. No declinations accepted.
- Influenza Vaccine: Recommended during flu season (Oct 1 - Mar 31). Exemptions accepted but mask wearing required if declined.
Health & Safety
- Fit Test: Required for all travelers; preferred before start but can be completed onsite. Specific mask types required in WI and MI.
- TB Testing: 2-step PPD within 1 year and 30 days of start or negative IGRA within 90 days of start. If positive, chest X-ray and TB questionnaire required.
- Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
- Drug Screen: 12-panel drug screen within 30 days prior to start, updated annually. Includes common substances such as amphetamines, cocaine, marijuana, opioids, etc.
- Positive PPD: Negative chest X-ray and annual TB questionnaire if applicable.
Professional Credentials
- Driver’s License/State ID: Must be valid, not expired, and address must match home state for compact license holders.
- Ascension Profile: Resume or professional profile submission required.
- Professional References: At least two qualifying professional references required prior to first assignment.
- Criminal Background Check: Includes Social Security trace, employment verification, OIG search, national sex offender check, and more. Must be completed within 30 days of first assignment and updated annually or after breaks in service.
- EPLS (Excluded Parties List System): Must be run within 90 days of start and rerun if break in service > 90 days.
- Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.
- Education Verification: Verification of education relevant to clinical work within 90 days of start. Official transcripts required if verification fails.
- Assignment Confirmation: Documentation of assignment start and end dates on agency letterhead required upon initial assignment and updates.
- Badge Photo: Forward-facing headshot with white or blue background, no alterations or filters.
Additional Notes
- Flu vaccine exemptions require mask wearing during flu season.
- Background checks and verifications must be maintained and updated regularly, especially after breaks in service longer than 90 days.
- Drug screening and health documentation are mandatory for onboarding and ongoing compliance.
This position requires strict adherence to Ascension Health’s credentialing and health compliance standards to ensure patient and staff safety.
