Job Description

Registered Nurse needed for the Pediatric Intensive Care Unit (PICU) at Ascension Saint Alexius Medical Center. This is a travel assignment located in Hoffman Estates, IL.


Required Certifications & Licenses

  • BLS (Basic Life Support) - Must be AHA certified
  • PALS (Pediatric Advanced Life Support) - Must be AHA certified
  • NRP (Neonatal Resuscitation Program) - AHA/American Academy of Pediatrics certified
  • Nursys Verification - Within 30 days prior to start; complete FACIS Questionnaire if disciplinary actions exist
  • Current Nursing License - Primary source verification within 30 days prior to start
  • Driver's License/State ID - Must be valid and match address requirements

Required Exams & Testing

  • Ascension Pharmacology Exam - Minimum 80% score, updated every 3 years
  • Ascension Specialty Exam - Minimum 80% or national benchmark, updated every 3 years
  • Ascension Color Vision Test - Completed prior to start and updated every 3 years
  • Ascension Core Competencies (HIPAA, Joint Commission, OSHA) - Completed within a year of start, updated yearly, minimum 80% score
  • Ascension EKG Exam - Passing score of at least 80%, updated as per latest edition
  • Ascension Cerner Test-Out - Required for electronic health record competency
  • Ascension Skills Checklist - Clinical skills checklist completed within one year of start, updated annually

Health & Immunization Requirements

  • MMR (Measles, Mumps, Rubella) - Documented proof of two vaccines or positive titer; no declinations accepted
  • Hepatitis B - Vaccination series completion or signed declination
  • Varicella - Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted
  • Tdap - Valid within 10 years; no declinations accepted
  • Influenza Vaccine - Optional; exemptions accepted but mask required during flu season (Oct 1 - Mar 31)
  • 2 Step PPD OR IGRA (TB Test) - Required within specified timeframes; if positive, chest X-ray and annual TB questionnaire required
  • Fit Test - Required annually for respirator use
  • Positive PPD - Negative chest X-ray and annual TB questionnaire required
  • Health Statement - Statement of good health or physical exam prior to assignment, updated annually
  • Ascension 12 Panel Drug Screen - Within 30 days prior to start, updated annually

  • Ascension Onboarding Requirements Education Attestation
  • Ascension Nursing Contract Worker Onboarding (done onsite)
  • Ascension Biometric Consent
  • Ascension Nova StatStrip (Pre-Hire Modules)
  • Ascension Profile (Resume)
  • Two Professional References
  • Criminal Background Check - Including SSN trace, employment verification, OIG search, national sex offender check, updated annually
  • Employment Verification - Past 7 years, updated annually
  • Education Verification - Must be completed within 90 days of start
  • Ascension Confirmation of Assignment - Provided upon initial assignment and updates for extensions
  • Ascension Badge Photo - Forward-facing headshot with white or blue background, no filters or alterations

Optional

  • Ascension Unit Specific Checklist - Optional for onboarding but must be uploaded within a week
  • Ascension Influenza Vaccine Exemption/Declination
  • Ascension Exit Survey - Completed at assignment end

This position requires strict adherence to Ascension Health’s credentialing, testing, and health requirements to ensure compliance and patient safety.
