Physical Therapist (PT) - Home Health

Location: Pittsfield, Massachusetts
Facility: Berkshire Visiting Nurse Association
Employment Type: Temporary
Contract Length: 13 weeks

Job Description

• Provide physical therapy services to home health clients.
• Deliver direct patient care and collaborate with the healthcare team.
• Participate in developing comprehensive patient care plans.
• Use rehabilitation equipment such as splints, orthoses, prostheses, wheelchairs, ADL equipment, and modalities.
• Must be Home Health Certified.

Requirements

• Experience:

  • Minimum 2+ years acute nursing care (hospital-based) and home care experience.
  • Acute care experience preferred.
  • Certified home health experience with proficiency in OASIS is required and non-negotiable.
• Licenses/Certifications:

  • Valid Massachusetts PT license.
  • BLS (AHA) certification.
• Other:

  • Valid driver’s license required.
  • Ability to work independently with minimal supervision.
  • Must provide at least two specific interview dates/times (no "anytime" availability accepted).
  • Cannot reside within 50 miles of the facility.
  • Submission must include work history, skills checklist (updated within 1 year), one supervisory signed reference (within past 2 years), and all licenses/certifications.
• COVID-19 Policy:

  • COVID vaccination (dose date on or after 10/3/23) required or approved exemption.
  • If exemption approved, mask must be worn at all times on site.

• Requested time off during assignment must be noted and verified at profile submission; no more than 5 days allowed.
• Non-billable orientation: 8 hours.
• Holiday and overtime multipliers apply.
• Call, call-back, and charge responsibilities are not applicable for this role.
