Job Description

• Maintain a professional image and provide excellent customer relations to patients, visitors, physicians, and co-workers in line with Service Excellence Standards and Core Values.
• Collaborate with staff to develop professional skills, new programs, and patient treatment plans as needed.
• Perform treatments as ordered, safely and timely, involving staff and family members when appropriate.
• Document medical records according to APTA standards of care.
• Participate in services, meetings, and educational sessions.
• Adjust schedules to accommodate changes in department caseload.
• Ensure a clean, orderly, and safe work environment.
• Evaluate patients referred for physical therapy using specialized testing and assessment of strength, range of motion, coordination, sensation, ambulation, transfers, bed mobility, and rehabilitation potential.
• Develop individualized physical therapy plans of care based on evaluation data, targeting short- and long-term goals.
• Supervise treatment team operations and regularly meet with PTAs to discuss patients and set goals.
• Provide evidence-based care and seek out research to support interventions.
• Maintain confidentiality of patient records and facility operations at all times.
• Adhere to all facility and corporate policies and procedures.

Required Credentials and Compliance

• BLS Certification (AHA only)
• Physical Therapist License/Certificate (state-specific registry verification required)
• Immunizations/Health Requirements:

  • MMR: Proof of 2 immunizations or titer with lab printout (no declinations accepted)
  • Hepatitis B: Proof of 3 vaccinations or titer with lab printout (no declinations accepted)
  • Varicella: Proof of 2 immunizations or titer with lab printout (no declinations accepted)
  • Influenza Vaccine: Required September through April (medical/religious exemptions considered case-by-case)
  • 2 Step PPD OR IGRA (Quantiferon/Tspot): First within 1 year, second within 30 days of start; negative Q-Gold or Tspot acceptable if within 90 days prior to start
  • Annual Negative PPD, Annual TB Questionnaire, Positive PPD with Negative Chest X-Ray (as applicable)
• Fit Test: Required for all travelers, completed within a year of start (optional)
• Driver's License/State ID: Valid and up to date
• Social Security Card: Copy required
• Criminal Background Check: Includes county, statewide, national search, OIG, EPLS, OFAC, NSOP, SS Trace, and education verification within 60 days of start covering last 7 years; updated annually
• Employment Verification: 5 years required
• Drug Screening: 10 panel drug screen completed within 30 days of start (rapid drug screen not accepted)
• Skills Checklist: Clinical skills checklist required prior to start and updated annually
• McLeod Profile: Resume or profile submission required; references preferred but not mandatory
• Confirmation of Assignment: McLeod standard form required
