Pediatric Physical Therapist - Travel/Contract

Location: Saint Cloud, Minnesota
Facility: CentraCare Plaza, CentraCare Health System MSP

Job Details

• Specialty: Therapy/Rehabilitation (Pediatric Physical Therapist)
• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Schedule: 4 shifts per week, 9 hours each (4x9)
• Weekend Requirement: Every 4th weekend (Saturday & Sunday)
• Shift Start: No earlier than 7:30 AM
• Shift End: No later than 6:00 PM
• No on-call duties
• Start Date: Any Monday in October (10/6, 10/13 preferred)

Requirements

• Minimum 1 year of experience as a Physical Therapist
• Minnesota license required (can submit application without having it in hand; temporary license issued quickly once documents are received)
• Must commit to applying for MN license once offered
• BLS certification required
• Accepts first-time travelers
• Must live at least 25 miles away from the facility (no local hires)
• Charting system: EPIC (experience preferred)
• Scrub color: Royal Blue

Certifications & Competencies

• BLS (Basic Life Support) required
• ACLS, PALS, NRP, and other certifications may be required depending on role (verify specifics if applicable)
• Competency exams required as applicable by profession, updated annually
• Medication/Pharmacology competency exam required for RNs (not applicable here but noted)

Health & Compliance Requirements

• Physical exam within 1 year of start
• Fit mask (N95) required
• Tdap vaccination required (no religious declinations accepted)
• Negative TB test or chest X-ray per MN state guidelines
• MMR (Measles, Mumps, Rubella) proof required (positive titer or 2 vaccines)
• Hepatitis B vaccination, positive titer, or declination accepted
• Varicella proof required (positive titer or 2 vaccines)
• Seasonal flu vaccination or declination required annually (mask required if declined)
• COVID-19 vaccine preferred; medical/religious exemptions accepted
• Drug testing not required but recommended within 1 year of start
• Background check within 1 year of start (7-year search, sex offender, nationwide, education verification)
• MN DHS NetStudy Fingerprint clearance required (no start with fingerprints pending)

Facility Specific Information

• Dress code must be reviewed prior to start
• Education modules (~16 hours) completed during orientation
• Confidentiality agreement must be signed prior to start
• Badge photo required (grey background, no selfies, JPG format)
• Candidates must return badges on last day of assignment
• COVID-19 policies and exemption forms apply

Additional Notes

• Candidates must complete MN DHS NetStudy Fingerprint Questionnaire immediately after signing contract to proceed with fingerprinting and clearance
• Fingerprinting must be completed within 2 weeks of registration
• Candidates not within driving distance of a livescan location must travel to complete fingerprinting before start date
