Job Opportunity: Physical Therapist Home Health
Location: Newton, Massachusetts
Facility: Mass General Brigham Home Care

Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Summary
- Provide physical therapy services to patients in their homes with physical impairments, functional limitations, or disabilities due to injury, disease, or other causes.
- Patient age range: birth to over 100 years.
- Collaborate with patients, families, physicians, and allied health professionals to assess needs and coordinate care.
- Maintain thorough and timely electronic documentation and interdisciplinary communication per agency policies and standards of practice.
- Weekend rotations occur once every 6 weeks.
- Ability to travel to assigned cases is required.
Requirements
- License: Must hold a valid Physical Therapist professional license in Massachusetts.
- Experience: Minimum 1 year of home care experience required.
Onboarding & Credentialing Requirements
Candidates must complete and submit the following prior to or at onboarding:
Professional Credentials
- MGB Badge Photo: Recent (within 6 months) professional headshot photo (no selfies, filters, eyeglasses; plain background).
- MGB MVR Check: Motor Vehicle Record check within 45 days of start (required for all home care positions).
- License/Certificate Verification: Primary source verification of professional license within 30 days of start. MA Conditional Licenses not accepted.
- Criminal Background Check: Within 45 days prior to start, including county and international searches if applicable.
- Massachusetts CORI Check: Criminal history check through MA CORI system within 30 days prior to start.
- National Sex Offender Check: Within 30 days of start and updated annually.
- Exclusion Checks: Screening against OIG, GSA, MassHealth, and NH Medicaid exclusion lists within 30 days of start and annually.
- Education Verification: Verification of highest education level achieved.
- SSN Trace: Address and alias history for past 7 years.
- Resume: Include past 7 years of work experience with city and state.
- References: Two recent references, at least one supervisor or above in the skillset.
- Skills Checklist: Required for submission.
- Candidate Submission Coversheet: Required for submission.
- Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.
Personal Health History & Immunizations
- Fit Test: N95 respirator fit test within 365 days of start, updated annually.
- MMR Vaccine or Immunity Proof: Documentation of measles, mumps, and rubella vaccination or immunity.
- TB Test: Negative TB skin test or blood test within 3 months of start, updated if >6 month break.
- Health Statement: Physician-signed health statement within 365 days of start, updated if >6 month break.
- MGB Nurse Aide Registry Check: Required by MA DPH for all employees within 30 days of start.
- Respirator Medical Clearance Attestation: Required if fit test is required.
- Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions allowed.
- Hepatitis B Vaccine or Immunity: Strongly preferred but optional.
- Tdap Vaccine: Preferred within last 10 years.
- Positive PPD - Negative Chest X-Ray: Required if positive TB test.
- TB Symptom Survey: Required if positive PPD, updated annually.
Additional Notes
- No religious exemptions are considered for fit testing or influenza vaccination.
- All documentation must comply with Mass General Brigham policies and Massachusetts Department of Public Health regulations.
- Candidates must be able to travel to patient homes as assigned.
