Long-Term Assignment (LTA)
Contract Length: 13 weeks


Required Credentials and Certifications

  • Basic Life Support (BLS)

    • AHA or ARC only
    • Required before onboarding
  • Advanced Cardiovascular Life Support (ACLS)

    • AHA only
    • Required before onboarding
  • Neonatal Resuscitation Program (NRP)

    • AHA only
    • Required before onboarding
  • AWHONN/Fetal Heart Monitoring Certification

    • Required before onboarding
  • Nursys License Verification

    • Nursing license verification within 30 days of start
    • Required before onboarding
  • Primary Source Verification of Professional License/Certification

    • Must be current and verified within 30 days of start
    • Licensure must be active in Louisiana for this assignment
    • Required before onboarding

Required Testing and Competencies

  • FMOLHS Skills Checklist

    • Unit-specific to scope of practice
    • Completed within 1 year prior to start and updated annually
  • FMOLHS Competency Exam

    • Passing score of 80% or higher within 1 year prior to start
  • FMOLHS Core Competencies

    • Includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives
    • Must be current within 1 year prior to start with passing score of 80% or higher
    • Updated annually
  • FMOLHS Contract Annual Education

    • Required annually

Health and Immunization Requirements

  • COVID-19 Vaccine

    • Optional; proof of vaccine or exemption accepted
  • Measles, Mumps, Rubella (MMR)

    • Proof of 2 immunizations, positive titer, or declination with signed statement
  • Hepatitis B

    • Proof of 3 immunizations, positive titer, or declination with required signatures
  • Varicella

    • Proof of 2 immunizations, positive titer, or declination with signed statement
  • Tetanus, Diphtheria, Pertussis (Tdap)

    • Proof of vaccination within past 10 years or declination with required signatures
  • Influenza Vaccine

    • Optional; medical or religious exemptions accepted with proper documentation
  • Tuberculosis (TB) Test

    • Negative QuantiFERON blood test within 1 year prior to start OR
    • 2-step TB skin test (1 within 30 days and 1 within 1 year)
    • If positive, MD note confirming no active TB or proof of treatment required
  • Annual TB Questionnaire

    • Required within 6 months of start
    • If positive, documentation of evaluation and treatment completion required
  • Fit Test

    • N95 mask fit test (3M 1860, 1860s, or 1870+) within 1 year prior to start
    • Updated annually
  • Health Statement

    • Signed and dated within 1 year of start confirming ability to work without restrictions
  • FMOLHS Eye Exam & Color Vision

    • Ishihara Color Test and Snellen Vision test within 1 year prior to start

  • Criminal Background Check

    • Completed within 30 days prior to start
    • Includes SSN address trace, county criminal searches for all counties lived/worked in last 7 years, national criminal check, education and employment verification (last 5 years), driving record
    • All reports must be uploaded together
  • Drug Screen

    • Negative 10-panel drug screen within 30 days prior to start
    • Includes testing for marijuana/THC, PCP, amphetamines, opiates, cocaine, benzodiazepines, barbiturates, methadone, propoxyphene, methaqualone
  • HRIS Form

    • Required for submission
  • Badge Photo

    • Forward facing, well-lit, non-blurry JPEG with neutral background
  • Code of Conduct

    • Signed and dated annually
