Job Requirements

• Location Restriction: Candidates with an Arizona home address on their driver’s license will not be considered.
• Experience: Minimum of 24 months current CVICU experience is required.
• Certifications:

  • Must have active American Heart Association (AHA) or American Red Cross certifications at time of submission.
  • Required certifications include:

    • ACLS (AHA)
    • BLS (AHA)
• Licensing:

  • Active Arizona RN license required at time of submission.
  • License verification must be less than 30 days old.
  • No license hits allowed.
• Vaccination:

  • Flu vaccination required; Banner declinations can be applied for (vaccination status must be included in submission notes).
• Background:

  • Banner has very specific background requirements that must be adhered to and attested.
• Travel Experience: Preferred but not mandatory.

Submission Requirements

• Complete Banner Cover Page with all requested information.
• Resume detailing all facilities worked at, including facility size, bed count, trauma level, and units.
• Skills checklist specific to the discipline, dated within the last year.
• Copy of valid driver’s license (non-AZ address).
• Certifications and license must be current and included in submission.
• All requested information must be 100% accurate.
• Return to Office (RTO) requests:

  • Must be less than 7 days.
  • Not approved during the first week of assignment or holidays.

Additional Notes

• Learning modules must be completed prior to assignment start.
• Non-billable hours are adjusted to 16 hours per assignment.
• Assignments may be canceled with 7 days written notice without penalty.
• Start date should be on or very soon after the order start date.
• Candidates should be pre-closed and ready to accept the assignment.
• All shifts, departments, and facilities the candidate is willing to work in must be clearly listed.
