Position Details

• Work Hours:

  • 12-hour shifts: 6:00 AM – 6:30 PM
  • 10-hour shifts: 8:00 AM – 6:30 PM
  • 9-hour shifts: 9:00 AM – 6:30 PM
• Weekly Hours: 36–40 hours plus call
• Call Requirements:

  • Overnight call approximately every 1–2 weeks
  • Weekend call approximately every 4–5th weekend
  • Holidays included in call rotation

Required Credentials & Onboarding Requirements

Licenses & Certifications

• Current RN license verification (dated within 30 days of start)
• BLS (American Heart Association) certification (front and back of card required)
• NIH Stroke Scale (NIHSS) certification
• Specialty-specific competency testing (exam within the last year)

Health & Immunizations

• Fit Test
• Trinity 10 Panel Drug Screen (9-panel, no THC; completed within 30 days of start, preferred within 14 days)
• COVID-19 vaccine with September 2024 booster, or approved declination/exemption (declination not accepted in CA or IL)
• MMR: Proof of 2 vaccinations or IgG titer lab report
• Hepatitis B: At least first vaccination prior to start; full series required (titers not accepted)
• Influenza vaccine (optional but recommended): Proof or declination/exemption (declination not accepted in CA or IL)
• 2 Step PPD or IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (per regional requirements)
• Varicella: Proof of 1 vaccination or IgG titer lab report
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work
• Tdap vaccine: Current and non-expired; declination accepted if no pediatric interaction

• Trinity Code of Conduct Form (signed)
• Trinity I9 form (completed by agency)
• Confidentiality and Information System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)

• Criminal Background Check (federal, nationwide, county for last 7 years; SSN verification; dated within 30 days)
