Job Description

• Position: MRI Technologist only (no CT or X-ray coverage required)

Required Credentials and Compliance for Onboarding

Licenses & Certifications

• BLS (Basic Life Support): AHA or ARC only, required for onboarding
• ARRT Certification: Required for onboarding
• Professional License: Active and verified for the State of Louisiana (Primary Source Verification within 30 days of start)

Immunizations & Health Requirements

• COVID-19 Vaccine: Optional (proof of vaccine or exemption accepted)
• MMR: Proof of 2 immunizations, positive titer, or declination with signed statement
• Hepatitis B: Proof of 3 immunizations, positive titer, or declination with required signatures
• Influenza Vaccine: Optional; medical or religious exemptions accepted with proper documentation
• TB Test: Negative QuantiFERON blood test within 1 year or 2-step skin test; if positive, MD clearance or treatment proof required
• Varicella: Proof of 2 immunizations, positive titer, or declination with signed statement
• Tdap: Proof of vaccination within past 10 years or declination with required signatures
• Annual TB Questionnaire: Required within 6 months of start; evaluation and treatment proof if positive
• Health Statement: Signed and dated within 1 year confirming ability to work without restrictions
• Fit Test: N95 mask fit test (3M 1860, 1860s, or 1870+) within 1 year, updated annually

Drug Screening & Background Checks

• Drug Screen: Negative 10-panel drug screen within 30 days prior to start
• Criminal Background Check: Completed within 30 days prior to start, including SSN trace, county and national checks, education and employment verification (last 5 years), driving record, and various federal databases

Competency & Skills Testing

• Skills Checklist: Unit-specific, within 1 year prior to start, updated annually
• Competency Exam: Passing score of 80% or higher within 1 year prior to start
• Core Competencies: Includes HIPAA, Safety, Infection Control, Cultural Diversity, etc., current within 1 year, passing score 80%+, updated annually

Additional Requirements

• FMOLHS Resume: Required for submission
• FMOLHS HRIS Form: Required for submission
• Badge Photo: Forward-facing, well-lit, neutral background, JPEG format
• Annual Education: Required as per FMOLHS guidelines
• Eye Exam & Color Vision: Ishihara Color Test and Snellen Vision test within 1 year prior to start
• Code of Conduct: Signed and dated annually
