Job Summary

• Provide direct nursing care following established policies, procedures, and protocols.
• Implement and monitor patient care plans.
• Monitor, record, and communicate patient condition appropriately.
• Coordinate all disciplines for well-coordinated patient care.
• Follow physician and nursing orders.
• Assess and coordinate patient discharge planning with healthcare team members.
• Requires critical thinking, decisive judgment, and ability to work with minimal supervision in a stressful environment.

Required Certifications & Licenses

• BLS (AHA certified)
• ACLS (AHA certified)
• Current nursing license with primary source verification within 30 days prior to start
• Nursys verification within 30 days prior to start
• Ascension Specialty Exam (completed prior to first assignment, updated every 3 years, minimum 80% passing score)
• Ascension Color Vision Test (completed prior to start, updated every 3 years)
• Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start, updated yearly, minimum 80% score)
• Ascension EKG Exam (new edition as of Dec 1, 2023; passing score of at least 80%, two attempts allowed)
• Ascension Onboarding Requirements Education Attestation
• Ascension Nursing Contract Worker Onboarding
• Ascension Nova StatStrip (pre-hire modules for non-allied positions)
• Ascension Cerner Test-Out (Cerner test results and training checklist with signed attestation)

Health & Immunization Requirements

• MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted. If negative, two doses of MMR vaccine four weeks apart required.
• Fit Test: Required for all travelers; preferred prior to start, can be completed onsite; updated annually.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Optional; flu season Oct 1, 2024 - Mar 31, 2025; declinations accepted with mask requirement.
• Positive PPD/Negative Chest X-Ray and Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest X-ray within a year showing no infection, and completed TB questionnaire. Annual updates required.
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): 1st PPD within 1 year, 2nd within 30 days of start, or negative IGRA within 90 days prior to start. Annual TB questionnaire updates required.
• Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted.
• Tdap: Valid within 10 years; no declinations accepted.
• Ascension 12 Panel Drug Screen: Completed within 30 days prior to start, updated annually; includes testing for 12 specified drugs with MRO review.

Professional Requirements

• Driver's License/State ID: Must be current, match address on file, and comply with compact license rules if applicable.
• Ascension Profile: Resume or professional profile required.
• Ascension 2 Professional References: At least two professional references completed prior to first assignment.
• Ascension Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender check; completed within 30 days of first assignment and updated annually.
• Ascension EPLS: Must be run within 90 days of start and rerun if break in service > 90 days.
• Ascension Skills Checklist: Clinical skills checklist completed within one year of start and updated annually.
• Ascension Education Verification: Verification of education relevant to clinical work within 90 days of start; rerun if break in service > 90 days.
• Ascension Confirmation of Assignment: Provided upon initial assignment and updated with each extension.

Additional Notes

• Some optional requirements include Ascension Unit Specific Checklist and Ascension Badge Photo.
• Post-assignment evaluations and exit surveys may be required.
• Legal documentation of name change must be provided if applicable.
